茚达特罗是新一代超长效β2受体激动剂.大量的临床前试验及临床试验证实茚达特罗起效迅速,在体内5 min即可产生支气管舒张效应,作用持续时间长达24 h,每日只需用药一次即能良好控制慢性阻塞性肺疾病和支气管哮喘患者喘息症状及改善肺功能.另外,茚达特罗引起的全身系统不良反应少,程度轻微,是治疗慢性阻塞性肺疾病和支气管哮喘的理想药物. 新一代超长效支气管扩张剂——茚达特罗(indacaterol) 一项对998名中重度慢性阻塞性肺疾病(COPD)患者进行的随机双盲临床研究发现，正在研究中的超长效吸入性β2受体激动剂茚达特罗(indacaterol)的疗效优于沙美特罗和安慰剂。 研究发现，在12周和26周时，每日1次茚达特罗可提供持续24 h的支气管扩张作用，优于每日2次的沙美特罗或者安慰剂。   此外，Oliver Kornmann博士在美国胸科医师学会年会上报告说：与其他两种治疗方式相比，接受茚达特罗治疗的患者呼吸困难减轻很多，对抢救性药物的需要显著减少，生活状况也有很大改善。   德国美因茨大学医学院附属医院的肺科医生Kornmann博士说，INLIGHT 2(评价150μg茚达特罗对COPD患者疗效-2)结果显示，中重度COPD患者通过单剂量干粉吸入器吸入茚达特罗是维持治疗的较佳选择。   此项研究的主要终点是第12周时的第1秒用力呼气量(FEV1)，茚达特罗组比沙美特罗组多60ml，比安慰剂组多170ml。Kornmann博士提到，尽管这些差异具有显著统计学意义，但预设的有临床意义的差异是120ml。   一个重要的次要终点是用圣乔治呼吸问卷表评价的健康状况。在第12周，与基线相比，茚达特罗组的总分比安慰剂组高出6.3分，比沙美特罗组高出2.1分，沙美特罗的用法是50μg，每日2次，用专用的干粉吸入器吸入。茚达特罗组患者改善了4分及以上者占58%，沙美特罗组和安慰剂组分别是47%和39%。   关于出现有临床意义的健康状况改善的几率，茚达特罗组是沙美特罗组的1.6倍，是安慰剂组的2.4倍。   在26周的随访中，茚达特罗组有60%的天数里不需要抢救性沙丁胺醇治疗，显著优于沙美特罗组的55%和安慰剂组的42%。   第12周时，以短暂呼吸困难指数总分改善至少1分作为指标，60%的茚达特罗治疗者出现了有临床意义的呼吸困难改善，这仅见于51%的沙美特罗组患者和40%的安慰剂组患者，结果有显著性差异。   严重不良事件的发生率在茚达特罗组是8.8%，沙美特罗组是5.7%，安慰剂组是7.8%。   COPD病情恶化在茚达特罗组患者是18%，在沙美特罗组是15%，在安慰剂组是19%，尽管这种恶化大多数为轻至中度。   值得注意的是，在基于ECG QT间期延长超过450ms(男性)和470ms(女性)方面，茚达特罗组为5.2%，沙美特罗组为1.8%，而安慰剂组为3.3%。   在细菌或者病毒性上呼吸道感染发生率方面，茚达特罗组是7.2%，沙美特罗组是1.8%，而安慰剂组是3.6%。   所有INLIGHT 2的参与者平均年龄63岁，都至少有20包•年的吸烟史。   此项研究由诺华公司资助，Kornmann博士是该公司的一名顾问。 Abstract Background: Indacaterol is a novel, once-daily β2-agonist in development for the treatment of asthma and chronic obstructive pulmonary disease. Studies were required to determine optimal dose(s) for continuing investigation. Objective: A dose-ranging study was undertaken to evaluate efficacy and safety of indacaterol. Methods: A total of 436 patients with persistent asthma receiving inhaled corticosteroids were randomized to 7 days treatment with once-daily indacaterol 50, 100, 200, or 400μg via multi-dose dry-powder inhaler (MDDPI; Certihaler™), indacaterol 400μg via single-dose dry-powder inhaler (SDDPI), or placebo. Serial 24-h spirometry was performed on days 1 and 7. Vital signs, laboratory evaluations, and adverse events were monitored. Results: All doses of indacaterol increased the mean time-standardized area under the curve of forced expiratory volume in 1s (FEV1) from 22 to 24h postdose (P≤0.001 vs placebo) on days 1 and 7, with clinically relevant treatment-placebo differences of 240, 260, 350, 300, and 380ml on day 1 and 230, 220, 320, 250, and 270ml on day 7 for indacaterol 50, 100, 200, and 400μg via MDDPI and 400 μg via SDDPI, respectively. All doses increased mean FEV1 (P<0.05 vs placebo) from 5min to 24h postdose on days 1 and 7. All doses were well tolerated. Most adverse events were mild-to-moderate in severity: most frequently reported were respiratory, thoracic, and mediastinal disorders. Conclusion: Once-daily dosing with indacaterol provided sustained 24-h bronchodilation in patients with moderate-to-severe asthma, with a satisfactory overall safety profile. Indacaterol 200μg appears the optimum dose, offering the best efficacy/safety balance.
