2018年3月24日，由盐野义制药（Shionogi）开发的新抗流感药物Xofluza（baloxavir marboxil，ゾフルーザ，曾用名S-033188）在日本得到了加速批准上市。为更多流感患者带来新希望。

药用药理学
1.体外病毒生长抑制作用
在A型和B流感病毒实验室菌株或临床分离感染的MDCK细胞（包括NA / H274Y突变体显示出降低的敏感性，以神经氨酸酶抑制剂）（犬肾来源的细胞系），Barokisabiru Marubokishiru活性形式显示出病毒生长抑制作用。[浓度抑制病毒滴度1/10（EC90）为0.46-0.98nmol/L，B型，在A型2.21-6.48nmol/L]
另外，这种效果，H5N1或H7N9亚型（NA / H274Y表现出降低的敏感性神经氨酸酶抑制剂，包括每个突变体NA/R292K）禽流感病毒也被在感染MDCK细胞中观察到。（EC 90：0.80至3.16nmol/L）
2.体内抗病毒作用
与类型A和B型流感病毒实验室菌株或临床分离株接种的小鼠模型小鼠肺内病毒后（包括NA / H274Y突变体显示出降低的敏感性，以神经氨酸酶抑制剂），Barokisabiru Marubokishiru给药天滴度以剂量依赖性方式降低。此流感接种效应模型小鼠中的病毒株抑制免疫功能，进一步，还在与禽流感病毒株（H5N1，H7N9）接种小鼠模型中观察到。
此外，与类型A和B型流感病毒株或禽流感病毒株（H5N1，H7N9）接种小鼠致死性模型中，Barokisabiru Marubokishiru改善死亡率14）。与接种流感病毒株，（给药开始病毒接种后24至96小时）的小鼠模型的治疗效果，也观察到延迟处理开始。
与接种流感病毒株雪貂模型，Barokisabiru Marubokishiru降低病毒滴度施用，在体温抑制增加第二天洗鼻。
3.作用机制
valoxabile marboxylic activator选择性地抑制A型和B型流感病毒的帽依赖性内切核酸酶活性。帽依赖性核酸内切酶是裂解特异性宿主细胞来源的mRNA前体，以产生必要的病毒mRNA合成一个引物的RNA片段的酶。 valoxabile羧酸活化剂通过抑制帽依赖性内切核酸酶活性和抑制病毒mRNA的合成而发挥病毒生长抑制作用。
4.抵抗
在III期临床试验的国内针对儿童超过12岁以下，在患者用此药已经施用，77例患者中18例患者核苷酸序列分析给药前后（可能是要么A型流感病毒感染患有疾病的患者在聚合酶酸性蛋白质区域具有I38氨基酸突变，其是valoxabile marboxylic activator的结合靶位点。在成人和12岁以上儿童的国际合作III期临床试验中，患者用此药已被给予370案件移出36案件序列分析之前和管理（A流感病毒后有可能传染性患者）接受氨基酸突变I38，其中在合并感染的患者一例A类和B型流感病毒时，观察到在这两种类型的I38的氨基酸变异。在任何的临床试验，在该药物的患者群中检测的氨基酸突变I38，采用在从该药物的给药3天观察到病毒效价的瞬时增加。顺便提及，在氨基酸突变的病毒滴度的变化存在的国际联合III期临床试验I38，在成人治疗的患者中观察到这种药物，并在图5中示出年龄大于12岁的儿童这是。
在体外用A型和B型流感病毒分离株的实验室测试耐药株，所述病毒株，以Barokisabiru Marubokishiru活化剂的病毒株灵敏度以最大相比于亲本菌株以获得降低大约100倍在这些菌株中，观察到I38T氨基酸突变。此外，氨基酸突变病毒显示培养细胞的增殖能力降低。另一方面，B型病毒株中未分离出氨基酸突变。

适应症
甲型流感或乙型流感病毒
用法与用量
1.对于12岁及以上的成人和儿童，通常口服2或20mg片剂或4粒胶囊（40mg作为valoxabile maloxil）。 然而，对于体重为80kg或更高的患者，口服口服4或20mg片剂或8粒胶囊（valoxabile maloxil为80mg）。
2.对于12岁以下的儿童，以下剂量应按如下方式口服给药。

Description
This medicine inhibits influenza viral growth in cells, thereby relieving symptoms of influenza.
It is usually used for the treatment of type A or type B influenza infection.

Effect/Efficacy
This medicine inhibits influenza viral growth in cells, thereby relieving symptoms of influenza.
It is usually used for the treatment of type A or type B influenza infection.
Usage/Dosage
In general, for adults and children at age of 12 years old or more, take 40 mg as active ingredient at a time, in a single dose. For these with body weight of 80 kg or more, take 80 mg at a time, in a single dose.
In general, for children at age of 12 years old or less, take 40 mg at a time for these with body weight of 40 kg or more, take 20 mg at a time for these with body weight of 20 to 40 kg, take a tablet (10 mg) at a time for these with body weight of 20 kg or less, in a single dose in each case.
Strictly follow the instructions.
If you accidentally take more than your prescribed dose, consult with your doctor or pharmacist.
Do not stop taking this medicine unless your doctor instructs you to do so.
Cautions
If you have previously experienced any allergic reactions (itch, rash, etc.) to any medicines.
If you are pregnant or breastfeeding.
If you are taking any other medicinal products. (Some medicines may interact to enhance or diminish medicinal effects. Beware of over-the-counter medicines and dietary supplements as well as other prescription medicines.)
If you are breastfeeding, avoid it while taking this medicine.
Keep the medicine out of the reach of infants and children. Store it at room temperature (1 to 30 degrees Celsius) away from direct sunlight and moisture.
Discard the remainder. Do not store them.
[To parental guardians etc.] There have been some reports on the onset of psychoneurotic symptoms such as abnormal behavior in patients treated with anti-influenza virus medicines. Therefore, you should keep the following in mind. There is a risk of occurrence of abnormal behavior in children or minors. In case of recuperation at home after the treatment with this medicine, the parental guardians etc. should make sure that the child or the minor is not left alone for at least 2 days.
Contraindication
The most commonly reported adverse reactions include diarrhea. If any of these symptoms occur, consult with your doctor or pharmacist.
The symptoms described below are rarely seen as initial symptoms of the adverse reactions indicated in brackets. If any of these symptoms occur, stop taking this medicine and see your doctor immediately.
