*新型单根皮下埋植剂Implanon(etonogestrel implant)

美国FDA近日批准了68mg的Implanon (etonogestrel、依托孕烯) 皮下植入棒式避孕药， 其植入后可维持三年的避孕作用。Implanon是用一种柔软的医用聚合材料制成的，植入上臂内侧，其可持续低量稳定的释放依托孕烯达到避孕作用。于月经周期的前5天皮下埋植。Implanon植入后几乎是不可见的，根据使用者的要求可随时取出，取出后使用者可恢复正常受孕。

由于Norplant的月经失调副反应和取出困难，英国自1998年已不再使用。Norplant于1990年被美国FDA批准使用，2002年己从市场撤出。此装置包括6个植入棒，共含有216mg左炔诺孕酮，供使用5年后取出。较新的研究集中在减少植入棒数及使用较少雄激素作用的孕激素和研究可降解的埋植剂，如Capronor，炔诺酮颗粒均是可生物降解的埋植物，但尚处于研究之中。NorplantⅡ，已被FDA批准使用，但尚未在美国上市。它包括了2根4cm长的植入棒，共含有150ug左炔诺孕酮。使用期3年，失败率为0．8-1.0／100妇女年。这种醋酸乙烯基乙烯的共聚物比Norplant中的硅胶材料更稳定。

现研究出一种非生物降解的单根皮下埋植剂Implanon，释效3—酮—地索高诺酮(ENG)，也称依托孕唏 (etonogestrel)。棒长4mm，直径2mm，埋植剂药芯含68mg晶体ENG，埋植后3个月吸收率为60ug／天，两年末为30ug／天。避孕机制主要是抑制排卵和宫颈粘液变稠厚、阻碍精子穿透。避孕效果：据多国家多中心研究，2362例妇女使用73429个周期(5629个妇女年)无一例妊娠。102例体重≥70kg的妇女使用2-3年亦无妊娠发生。取出后3个月几乎完全恢复正常月经，94％证实恢复排卵。副反应主要为月经改变，流血天数90天占17．1％，闭经率20％-39％，月经稀发率29％—51％。其他如头痛、恶心、乳房胀痛、情绪变化、体重增加、痤疮等，与Norplant相似。埋植前有痛经者85％—88％使用中痛经消失或减轻。

Some important information about Implanon
The subdermal implant marketed as Implanon has now been replaced by a radio-opaque version with a new insertion mechanism designed to reduce the risk of deep insertion.

The new implant is called Nexplanon. Like Implanon, the active hormone contained in Nexplanon is etonogestrel 68mg. It is inserted sub-dermally and is bioequivalent to Implanon.

Nexplanon is different from Implanon in two important ways. Firstly, the implant is radio-opaque and can therefore now be located using x-ray or CT scan, if necessary. The major change is a newly designed preloaded applicator, which has been designed to reduce the risk of deep insertion or non-insertion. The removal technique remains unchanged.

Implanon is a 4cm long flexible tube, about the size of a hairgrip.  Placed under the skin of the upper arm, Implanon steadily releases 68mg of etonogestrel into the bloodstream over a three year period.
