Myrin-P Forte(Ethambutol HCl, rifampicin, INH, pyrazinamide)
Myrin®-P复地是一个固定剂量组合(FDC)含有利福平(RMP,150毫克),异烟肼(INH,75毫克),乙胺丁醇(EMB)盐酸盐(275毫克)和吡嗪酰胺(PZA,400毫克)开发用于治疗结核病的(TB)。

Indications Initial phase treatment & re-treatment of all forms of TB in category I & II patients caused by susceptible strains of mycobacteria.

Dosage >71 kg 5 tab/day, 55-70 kg 4 tab/day, 40-54 kg 3 tab/day. Adult Category I patients Doses to be given daily during the initial phase (8 wk) of treatment. Category II patients Doses to be given daily during the initial phase (12 wk) of treatment. Treatment is continued for further 4 wk in both patients if sputum is smear-positive.

Overdosage View Myrin-P Forte overdosage for action to be taken in the event of an overdose.
Administration Should be taken on an empty stomach (Take 1 hr before or 2 hr after meals. May be taken w/ meals or antacids to reduce GI discomfort. Do not take w/ Al-containing antacids.).
Contraindications Hypersensitivity. Alcoholism, optic neuritis, impaired hepatic function, severe renal insufficiency, hyperuricemia, gouty arthritis, jaundice, retrobulbar neuritis. Patients who are unable to appreciate & report visual side effects or changes in vision eg young childn & patients w/ mental illness or deficiency. Patients <40 kg. Lactation. Pyrazinamide: Severe hepatic damage. Concurrently taking medications associated w/ liver injury, drink excessive amounts of alcohol, underlying liver disease or history of INH-associated liver injury.

Special Precautions Perform baseline & periodic assessment of hepatic function, blood uric acid. Preexisting liver disease or at increased risk for drug-related hepatitis (eg alcohol abusers). Discontinue pyrazinamide when signs of hepatocellular damage occur. History of gout. Not for use in latent TB infection. Pyridoxine deficiency. Full ophth exam before & during treatment. Discontinue if hyperuricemia is accompanied by acute gouty arthritis. Red-orange discoloration of urine, feces, sputum, saliva & tears. Soft contact lenses may be permanently stained. Produces pink-brown color w/ Acetest & Ketostix. False +ve reactions w/ copper sulfate urine glucose tests. Periodic assessment of organ system functions including renal, hepatic & hematopoietic should be made during therapy. Patients at risk for peripheral neuropathy, epilepsy, DM, decreased renal function. May impair ability to drive or operate machinery. Pregnancy. Childn <13 yr. Elderly.

Adverse Drug Reactions Leukopenia, thrombocytopenia, hypersensitivity syndrome, eosinophilia, fever, lymphadenopathy; anorexia, elevations of serum uric acid conc; dizziness, paresthesia; epigastric distress, constipation, nausea, vomiting, anorexia; hepatic impairment, jaundice; pruritus, rash, acute gout; malaise; headache; mental confusion; hepatitis or hepatitis prodromal syndrome. Ethambutol: Neutropenia; anaphylactic/anaphylactoid reaction, hypoesthesia; decreases in visual acuity, irreversible blindness; abdominal pain, metallic taste, dry mouth; Stevens-Johnson, toxic epidermal necrolysis; joint pain, pulmonary infiltrates w/ or w/o eosinophilia. Rifampicin: Hemolytic anemia, anemia; drowsiness, fatigue, ataxia, inability to conc, muscular weakness, generalized numbness, pain in the extremities; visual disturbances, exudative conjunctivitis; heartburn, diarrhea, cramps, sore mouth & tongue, fungal growth; shock-like syndrome w/ hepatic involvement, abnormal liver function tests; urticaria, pemphigoid reaction, purpura; elevations in BUN, interstitial nephritis, alteration in kidney function; menstrual disturbances; flu-like syndrome. INH: Agranulocytosis, methemoglobinemia, vasculitis; lupus-like or rheumatoid syndrome; peripheral neuropathy; insomnia, restlessness, toxic psychoses, increased reflexes, muscle twitching, convulsions; optic neuritis & atrophy; tinnitus; pellagra, hyperglycemia, metabolic acidosis, gynecomastia; exfolitive dermatitis; arthritic symptoms; prostatic obstruction syndrome, urinary retention; dry mouth. Pyrazinamide: Sideroblastic anemia w/ erythroid hyperplasia, vacuolation of erythrocytes, abnormalities in blood clotting mechanisms, porphyria; acne, photosensitivity; dysuria, interstitial nephritis; arthralgia, myalgia.

Drug Interactions Neurotoxic drugs, Al-containing antacids, phentolamine, coumarin-type drugs, OC, aminophylline, theophylline, sulfonylurea, oral antidiabetic agents, phenobarb, systemic β-adrenergic blocking agents, chloramphenicol, clofibrate, corticosteroids, cyclosporin, dapsone, diazepam, digitalis glycosides, disopyramide, mexiletine, quinidine, tocainide, estramustine, fluconazole, methadone, phenytoin, trimethoprim, oral verapamil, clofazimine, folate & vit B12, probenecid, alfentanil, inandione derivative anticoagulants, benzodiazepines, carbamazepine. Paracetamol, alcohol & rifampicin & hepatotoxic medications. Disulfiram, cycloserine, enflurane, ketoconazole or parenteral miconazole, glucocorticoid corticosteroids. Other antituberculous medications, gout prep. Cheese eg Swiss or Cheshire; fish eg tuna or sardines.
