性质
安平-10%为含有20种左旋结构氨基酸的溶液。氨基酸的模式是在临床状况下，根据药物动力学研究而制定，故可满足肝功能衰竭状态下的特殊代谢需要。肝功能衰竭是以氨基酸失调，尤其是支链氨基酸与芳香氨基酸之间的不平衡为特征，安平—10%的模式特别照顾到病人这些变化。
适应证
预防和治疗肝性脑病
肝病时肝性脑病急性期或表现期的静脉营养。
剂量
成人：除特别情况时可达15毫升/公斤体重/天外，
推荐平均剂量为7—10毫升/公斤体重/天
滴速：1毫升/公斤体重/小时
对昏迷病人，治疗的最初数小时滴速可加快：
第一至第二小时：2毫升/公斤体重/小时
第三至第四小时：1.5毫升/公斤体重/小时
从第五小时开始：1毫升/公斤体重/小时
给药途径： 经中央静脉输注
禁忌证
非肝原性的氨基酸代谢絮乱肾功能衰竭伴病理性非蛋白氮酸中毒
水潴留
休克
副作用
如输注速度过快，可能出现不耐受
注意
应密切注意水，电解质和酸一碱平衡。根据血清离子谱补充电解质
为支持输入氨基酸参与合成代谢，达到最好利用，能量物质（葡萄糖和脂肪）应同时输入。一般情况下，患严重肝病的成人非蛋白热卡的供应为128-149千焦尔（30-35千卡）/公斤体重/天。
将氨基酸溶液与其它液体或药物混合，会增加理化性不相容和微生物污染的危险，混合过程应在无菌条件下进行，并且混合物之间应是相容的。
氨基酸治疗并非用以取代经证实的治疗肝性脑病的方法，例如净化性治疗，使用乳果糖和/或肠道抗菌素杀菌。

Aminoplasmal® Hepa-10%
Indications
Prophylaxis and therapy of hepatic encephalopathy.
Parenteral nutrition in liver diseases when hepatic encephalopathy is either imminent or already manifest.
Normalisation of amino acid imbalances arising from severe liver diseases.
Dosage
Unless otherwise prescribed, depending on the individual requirements.
Normal dose : 7 - 10 ml/kg body weight/day, 0.7-1.0 g of amino acids/kg body weight/day
Maximum dose : 15 ml/kg body weight/day, 1.5 g of amino acids/kg body weight/day
Flow rate
Maintenance requirements in parenteral nutrition:Up to 1.0 ml/kg body weight/h or (for a 70 kg patient): up to 25 drops/min corresponding to approximately 70 ml/h
Treatment of hepatic coma: In comatose patients it is recommended to administer Aminoplasmal® Hepa-10% at the beginning of the treatment, until the effect sets in, more rapidly than stated above, for example for a 70 kg patient:1st - 2nd h : 50 drops/min corresponding to approximately 150 ml/h3rd - 4th h : 25 drops/min corresponding to approximately 75 ml/h
from 5th h : 15 drops/min corresponding to approximately 45 ml/h
Route of administration
I.V. via a central venous catheter
Contraindications
Disturbances of amino acid metabolism of other than hepatic origin, acidosis, overhydration, hypokalaemia.
Precautions
Clinical supervision should include regular checks of fluid balance and serum electrolytes. Carbohydrates and electrolytes are to be supplemented as required.
Caution is to be exercised in patients with hyponatraemia or with increased serum osmolarity. Unusally rapid infusion rates may produce signs of intolerance and renal amino acid losses resulting in amino acid imbalances.
In cases of concurrent renal insufficiency the dosage of amino acids has to be adapted to the serum urea and creatinine values.
Amino acid therapy is not a substitute for established therapeutic measures, such as purging, administration of lactulose and/or gut sterilising antibiotics, in the treatment of hepatic encephalopathy.
It is not recommended that any additives be incoporated into Aminoplasmal® Hepa-10% solution, but should preferably be given in standard carbohydrate or electrolyte solutions. However, if admixture with Aminoplasmal® Hepa-10% is essential, then the compatibility of the additive must be checked before administration.
Side-effects
Provided contraindications, dosage recommendation and precautions are observed, side-effects are not anticipated.
