美国眼科学会2003年会上提交一项研究说,莫西沙星(Vigamox)对细菌性结膜炎新生儿的耐受性很好,治疗成功率超过80%。
细菌性结膜炎是出生后头一月最常见的感染,约见于2-12%的新生儿。遗憾的是,目前FDA还没有批准用于1月以下新生儿局部应用的抗生素。
Vigamox(盐酸莫西沙星滴眼液,0.5%)是第四代氟喹诺酮药,对革兰氏阳性菌和阴性菌都有效。2002年春天该药可用于治疗3岁以上的细菌性结膜炎患者,但对更小的病人是否有效尚待研究。
美国犹他州山景眼科中心特利夫特(Robert L Treft)和同事评估了本药用于新生儿的安全性和有效性。该研究共纳入209名病儿,2-31天之间,随机分为接受Vigamox(0.5%)或Ciloxan(0.3%),每天三次,连续四天。
治疗初和第9天采集了眼分泌物并培养。开始68%的患儿培养物阳性,最常见的病原物为表皮葡萄球菌。培养物阳性的病人用Vigamox治疗的,病原物清除了92%,而且58%到治疗结束时达到临床治愈,到第9天时80%临床治愈。培养物阳性的病人用Vigamox治疗的第二天超过一半即有反应,到治疗结束时80%临床改善,第9天83%临床改善。发生副作用的很少,Vigamox组2人、Ciloxan组1人出现充血。另外后组还有一人眼皮水肿。

Uses

This medication is a quinolone antibiotic used for eye infections.

How to use

For best results, use exactly as directed for the full time prescribed. Stopping this medication too soon may result in a relapse of the infection.To apply eye drops, wash hands first. To avoid contamination, do not let the dropper tip touch any surface.Tilt your head back, gaze upward and pull down the lower eyelid to make a pouch. Place the dropper directly over eye and instill the prescribed number of drops. Look downward and gently close your eye for 1 to 2 minutes. Place one finger at the corner of the eye near the nose and apply gentle pressure. This will prevent the medication from draining away from the eye. Try not to blink and do not rub the eye. Do not rinse the dropper.If you are using other kinds of eye drops, wait at least five minutes before applying the other medications.Do not wear contact lenses while you are using this medicine. Sterilize contact lenses according to manufacturer's directions and check with your doctor before using them.Inform you doctor if your condition does not improve in 7 days.

Missed dose

If you miss a dose, use it as soon as remembered; do not use if it is almost time for the next dose, instead, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up.

Precautions

Before using this medication, tell your doctor or pharmacist if you are allergic to it; or to quinolone antibiotics (such as ciprofloxacin); or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.Before using this medication, tell your doctor or pharmacist your medical history, especially of: contact lens use.Your vision may be temporarily blurred or unstable after applying this drug. Do not drive, use machinery, or do any activity that requires clear vision until you are sure you can perform such activities safely.Use of this medication for prolonged or repeated periods may result in a secondary infection.This drug should be used only when clearly needed during pregnancy. Discuss the risks and benefits with your doctor.It is not known if this medication passes into breast milk. Consult your doctor before breast-feeding.

Side effects

Blurred vision, watery eyes, eye pain/dryness/redness/itchiness may occur. If any of these effects persist or worsen, inform your doctor.Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.Tell your doctor immediately if any of these unlikely but serious side effects occur: swelling of the eye.An allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching/swelling (especially of the face/tongue/throat), dizziness, trouble breathing.This is not a complete list of possible side effects. Overdose

An overdose with this product may be flushed from the eye(s) with warm tap water.

Notes

This medication has been prescribed for your current condition only. Do not use it later for another infection unless told to do so by your doctor. A different medication may be necessary in those cases.Do not share this medication with others.

Storage

Store this medicine in the refrigerator or at room temperature between 36 and 77 degrees F (between 2 and 25 degrees C) away from moisture and sunlight. Do not freeze. Do not store in the bathroom. Discard the solution if it changes color, turns cloudy, or if it contains particles. Keep all medicines away from children and pets.Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed. Consult your pharmacist or local waste disposal company for more details about how to safely discard your product.
