【中 间 体】1H-吲哚,(1H-吲哚-3-基)氧代乙酸乙酯,2-(1H-吲哚-3-基)乙醇,3-(2-溴乙基)-1H-吲哚,N-吡啶-4-基苯甲酰胺

【药理作用】 本品为α肾上腺素受体阻断剂。能选择性地竞争周围血管突触后的α受体,使周围血管平滑肌松弛,使血压下降。此外,本品还有局部麻醉作用,使心肌膜稳定性增加,故不产生类似其它。受体阻断剂的代偿反射性心率加快现象。
本品还具有膜稳定作用。口服可使血管舒张和血压降低,可单独或利尿药合用治疗轻、中度高血压,也可治疗偏头痛。
【临床应用】 用于各种类型的原发性高血压,也可试用于哮喘和偏头痛。
【剂量与用法】口服25mg一次,一日2次。
【毒副作用】常见的不良反应有口干、鼻塞、眩晕、抑郁、射精困难和皮疹。减量时,可自行消失。
【药物相互作用】本品不能与单胺氧化酶抑制剂合用,与其它降压药合用时宜减量。
【禁忌症】心力衰竭、肝肾功能不全、帕金森氏病、抑郁症、癫痫等患者禁用;老人慎用。
【用法用量】口服:初始25mg/d,分2次服用,必要时,剂量可加大,最大剂量200mg/d,分2—3次服用。如果每天各次给药剂量不等时,宜将最大剂量放在夜间服用,以避免白天打瞌睡。

Baratol (indoramin)

How does it work?
Baratol tablets contain the active ingredient indoramin hydrochloride, which is a type of medicine called an alpha blocker. It works by blocking alpha receptors in certain areas of the body.

Alpha receptors are found on the muscle in the walls of blood vessels. When indoramin blocks these receptors it causes the muscle in the blood vessel to relax and the blood vessel to widen. This lets the blood pass more easily through the blood vessels and hence reduces the pressure in the blood vessels. Indoramin can therefore be used to treat high blood pressure.

What is it used for?

High blood pressure with no known cause (essential hypertension).

(Indoramin is also used to treat the symptoms of an enlarged prostate gland. See the Doralese factsheet linked at the end of this article for more information about this other use of indoramin.)

Warning!

  • This medicine can occasionally cause your blood pressure to drop when you move from a lying down or sitting position to sitting or standing, especially when you first start taking the medicine.
  • This may make you feel dizzy or unsteady and may rarely cause fainting. To avoid this, take care when moving from lying down to sitting or standing, particularly if you wake up in the night - try getting up slowly. If you do feel dizzy, sit or lie down until the symptoms pass.
    This medicine may cause drowsiness and dizziness. If affected do not drive or operate machinery.
  • You should avoid drinking alcohol while taking this medicine, because it may increase the absorption of indoramin from the gut and could therefore increase its effects. If you do feel sleepy or dizzy while taking this medicine this is also likely to be made worse by drinking alcohol.
  • If you are scheduled to have eye surgery for cataracts it is important to let your eye specialist know if you are or have been taking this medicine. This is because this type of medicine may make your pupil dilate poorly and the iris (the coloured circular part of the eye) become floppy during the procedure. The specialist needs to know if you have been taking this medicine so they can take appropriate precautions with the medicines and techniques they use during the surgery. You may be asked to stop taking this medicine for a period of time before cataract surgery. Check with your doctor.

Use with caution in

  • Elderly people.
  • Decreased kidney function.
  • Decreased liver function.
  • Epilepsy.
  • Parkinson's disease.
  • History of depression.

Not to be used in

  • Heart failure.
  • People taking monoamine oxidase inhibitors (MAOIs - see end of factsheet for more details).
  • This medicine is not recommended for people who have ever fainted after passing urine (micturition syncope), or who suffer from drops in blood pressure that cause dizziness when moving from lying or sitting to standing (postural hypotension).
  • This medicine is not recommended for use in children.

This medicine should not be used if you are allergic to one or any of its ingredients. Please inform your doctor or pharmacist if you have previously experienced such an allergy.

If you feel you have experienced an allergic reaction, stop using this medicine and inform your doctor or pharmacist immediately.

Pregnancy and breastfeeding
Certain medicines should not be used during pregnancy or breastfeeding. However, other medicines may be safely used in pregnancy or breastfeeding providing the benefits to the mother outweigh the risks to the unborn baby. Always inform your doctor if you are pregnant or planning a pregnancy, before using any medicine.

  • This medicine is not recommended for use in pregnancy unless considered essential by your doctor. Seek medical advice from your doctor.
  • It is not known if this medicine passes into breast milk. It is not recommended for use during breastfeeding unless considered essential by your doctor. Seek medical advice from your doctor.

Label warnings
This medication may cause drowsiness. If affected do not drive or operate machinery. Avoid alcoholic drink.

Side effects
Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. Because a side effect is stated here, it does not mean that all people using this medicine will experience that or any side effect.

  • Drowsiness.
  • Dizziness.
  • Dry mouth.
  • Nasal congestion.
  • Headache.
  • Fatigue.
  • Weight gain.
  • Low blood pressure (hypotension).
  • A drop in blood pressure that occurs when going from lying down to sitting or standing, which results in dizziness and lightheadedness (postural hypotension).
  • Depression.
  • Problems with ejaculation.
  • Diarrhoea.
  • Nausea.
  • Increased need to pass urine.
  • Awareness of your heartbeat (palpitations).

The side effects listed above may not include all of the side effects reported by the drug's manufacturer.

How can this medicine affect other medicines?
It is important to tell your doctor or pharmacist what medicines you are already taking, including those bought without a prescription and herbal medicines, before you start treatment with this medicine. Similarly, check with your doctor or pharmacist before taking any new medicines while taking this one, to ensure that the combination is safe.

The manufacturer states that this medicine must not be taken in combination with monoamine oxidase inhibitor medicines (MAOIs). These include the following:

  • MAOI antidepressants such as phenelzine, tranylcypromine or moclobemide
  • the antibiotic linezolid
  • rasagiline and selegiline for Parkinson's disease.

If indoramin is taken with other medicines that reduce blood pressure, either as a treatment for high blood pressure (antihypertensives), or as a side effect, there may be an enhanced blood pressure lowering effect. This might make some people feel dizzy, particularly when you first start taking the indoramin. If you do feel dizzy, you should lie down until the symptoms pass. If any dizziness persists you should let your doctor know, as your medicine doses may need adjusting. Other medicines that can reduce blood pressure include the following:

  • ACE inhibitors, eg enalapril, captopril
  • other alpha-blockers, eg alfuzosin, doxazosin
  • anaesthetics
  • angiotensin II receptor antagonists, eg losartan
  • antipsychotic medicines
  • baclofen
  • benzodiazepines, eg temazepam
  • beta-blockers, eg propranolol
  • calcium channel blockers, eg diltiazem, verapamil, nifedipine
  • diuretics, eg furosemide
  • levodopa
  • moxisylyte
  • nitrates, eg glyceryl trinitrate, isosorbide mononitrate, amyl nitrate (poppers)
  • phosphodiesterase type 5 inhibitors for impotence, eg sildenafil, vardenafil, tadalafil
