【剂型】:口腔软膏剂
【主要成份】曲安奈德。
【性状】本品为浅棕黄色糊剂。每克含曲安奈德1毫克,由明胶、果胶、X甲具纤维素钠组成,是一种聚乙烯和矿物油的胶状基质。
【药理毒性】康宁乐是一种皮质类固醇,具有显著的抗炎、止痛及抗过敏作用,可以迅速缓解口腔疼痛,炎症和溃疡,康宁乐的基质具有粘附作用,可使药物与病损长时间紧密接触,保护覆盖创面,并使类固醇更好的发挥药效。
【适应症】口腔粘膜的急、慢性炎症,包括复发性口腔溃疡、糜烂甲扁平苔茴,口炎创伤性病损,如义齿造成的创伤性溃疡;剥脱性龈炎和口腔炎。创伤性溃疡和多数非复发性病损治疗后可迅速痊愈。慢性病变和复发性口腔溃疡用药后也能获得显著疗效,停药后还会复发。但康宁乐口内膏对控制复发也有一点作用。
【用法用量】挤出少量药膏(大约 1cm)轻轻涂抹在病损表面使之形成薄膜。在有些病损可能要多取一些药膏。最佳的结晶效果是药膏形成的薄膜恰好覆盖病损。不要反复揉擦。刚刚涂抹药膏时可能感觉药物呈颗粒和砂样,有些粗糙。药膏涂好后就可以形成光滑的薄膜。康宁乐最好在睡前使用,这样可以使药物与患处整夜接触。如果症状严重,需要每天涂2-3次。以餐后用药为宜。
【不良反应】对这种制剂不能耐受者非常少见。口腔内局部使用没有见到对皮质类固醇的反应。长期使用可引起如同全身使用类固醇类药物的副作用,如肾上腺皮质功能抑制,葡萄糖代谢改变、蛋白质分解和消化道溃疡复发等等。这些情况在激素停止使用后可以逆转和消失。
【禁忌】患者对康宁乐中任何一种成份有过敏史。由于康宁乐中含有皮质类固醇、禁用于口腔、咽部的真菌和细菌感染性疾病。由病毒引起的口腔疱疹。如:唇疱疹、疱疹性龈口炎、疱疹性咽峡炎等也不要使用康宁乐。
【注意事项】按规定剂量使用康宁乐类固醇用量很小,不太可能对全身产生影响。当然长期局部过量使用也会出现异常情况。如:乏力、头晕等,出现这些情况应该与医生联系。如果出现持续的局部刺激症状和过敏反应,应该立即停药并采取相应治疗。患有结核病、消化道溃疡和糖尿病的病人若无医嘱不能使用皮质类固醇类药物。必须牢记在患者接受皮质类固醇治疗时,口腔的正常防御反应受到抑制、口腔微生物的毒株会繁殖,且不出现通常的口腔感染征兆。用药7天后,如果病损没有显著修复、愈合时,建议做进一步检查。
【孕妇和哺乳期妇女用药】怀孕期妇女使用(C型)。一般怀孕妇女不要使用,除非内科医生和牙科医生认为根据患者病情使用利大于弊在动物实验中发现皮质类固醇能够引起流产和各种畸形,如:腭裂、骨形成障碍,当然这些情况不一定在人类出现。但是长期使用皮质类固醇可使胎盘和消声儿体重减轻,因为长期使用皮质类固醇会使新生儿肾上腺皮质受到抑制,所以孕妇使用该药应慎重,孕妇产前短期使用皮质类固醇预防新生儿呼吸窘迫综合征对孩子没有影响,有母亲出现肺水肿的报告。

What is Kenalog?
Triamcinolone is a topical steroid. It reduces the actions of chemicals in the body that cause inflammation, redness, and swelling.Kenalog is used to treat the inflammation caused by a number of conditions such as allergic reactions, eczema, and psoriasis. The dental paste form of Kenalog is used to treat mouth ulcers.Kenalog may also be used for other purposes not listed in this medication guide.

What is the most important information I should know about Kenalog?
Use Kenalog exactly as directed on the label, or as it has been prescribed by your doctor. Do not use the medication in larger amounts or for longer than recommended.

Do not cover treated skin areas with a bandage or other covering unless your doctor has told you to. If you are treating the diaper area of a baby, do not use plastic pants or tight-fitting diapers. Covering the skin that is treated with Kenalog can increase the amount of the drug your skin absorbs, which may lead to unwanted side effects. Follow your doctor's instructions.

Avoid using Kenalog on your face, near your eyes, or on body areas where you have skin folds or thin skin.

Do not use Kenalog on a child without a doctor's advice. Children are more sensitive to the effects of Kenalog.

Kenalog will not treat a bacterial, fungal, or viral skin infection.

Contact your doctor if your condition does not improve or if it gets worse after using Kenalog for several days.

What should I discuss with my healthcare provider before using Kenalog?
Do not use Kenalog if you are allergic to triamcinolone.

Before using Kenalog, tell your doctor if you are allergic to any drugs, or if you have:any skin infection,especially tuberculosis infection of the skin;chicken pox or herpes infection (including cold sores);diabetes; ora stomach ulcer.

If you have any of these conditions, you may not be able to use Kenalog, or you may need a dosage adjustment or special tests during treatment.

FDA pregnancy category C. This medication may be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment. It is not known whether Kenalog passes into breast milk or if it could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby. Do not use this medication on a child without a doctor's advice. Children are more sensitive to the effects of Kenalog.

How should I use Kenalog?
Use Kenalog exactly as it was prescribed for you. Do not use the medication in larger amounts, or use it for longer than recommended by your doctor. Follow the instructions on your prescription label.

If you are using the dental paste, apply the medication in a thin layer, just enough to cover the mouth ulcer. The paste may stick better if you dry the mouth ulcer before applying the medication.

Do not cover treated skin areas with a bandage or other covering unless your doctor has told you to. If you are treating the diaper area of a baby, do not use plastic pants or tight-fitting diapers. Covering the skin that is treated with Kenalog can increase the amount of the drug your skin absorbs, which may lead to unwanted side effects. Follow your doctor's instructions. Contact your doctor if your condition does not improve or if it gets worse after using Kenalog for several days. It is important to use Kenalog regularly to get the most benefit. Store Kenalog at room temperature away from moisture and heat.

What happens if I miss a dose?
Use the medication as soon as you remember. If it is almost time for the next dose, skip the missed dose and use the medicine at the next regularly scheduled time. Do not use extra medicine to make up the missed dose.

What happens if I overdose?
Seek emergency medical attention if you think you have used too much of this medicine, or if anyone has accidentally swallowed it. An overdose of Kenalog applied to the skin is not expected to produce life-threatening symptoms.

What should I avoid while using Kenalog?
Avoid getting Kenalog in your eyes, mouth, and nose, or on your lips. If it does get into any of these areas, wash with water. Do not use Kenalog on sunburned, windburned, irritated, or broken skin. Also avoid using this medication in open wounds.

Avoid using skin products that can cause irritation, such as harsh soaps or shampoos or skin cleansers, hair coloring or permanent chemicals, hair removers or waxes, or skin products with alcohol, spices, astringents, or lime. Do not use other medicated skin products unless your doctor has told you to.

Kenalog side effects
Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Stop using this medication and call your doctor at once if you have any of these signs that you may be absorbing Kenalog through your skin or gums: blurred vision, or seeing halos around lights;uneven heartbeats;mood changes;sleep problems (insomnia);weight gain, puffiness in your face; orfeeling tired.

Less serious side effects may include:skin redness, burning, itching, or peeling;thinning of your skin; or blistering skin; or stretch marks.

This is not a complete list of side effects and others may occur. Tell your doctor about any unusual or bothersome side effect. You may report side effects to FDA at 1-800-FDA-1088.

What other drugs will affect Kenalog?
It is not likely that other drugs you take orally or inject will have an effect on topically applied Kenalog. But many drugs can interact with each other. Tell your doctor about all your prescription and over-the-counter medications, vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start a new medication without telling your doctor.
