甲状腺激素类药。本品为人工合成的四碘甲状腺原氨酸钠,在体内转变成三碘甲状腺原氨酸(T3)而活性增强,具有维持人体正常生长发育、促进代谢、增加产热和提高交感-肾上腺系统感受性等作用。 药代动力学
经用131I标记的左甲状腺素钠研究证明,甲状腺功能正常时半衰期(t1/2)为7天,组织中为0.4天,停药后仍可维持药效数周。口服后平均吸收50%,绝大部分与血浆蛋白结合,约80%与甲状腺素结合球蛋白结合。主要在肝中代谢,大部分由尿中排泄。 适应症
适用于先天性甲状腺功能减退症(克汀病)与儿童及成人的各种原因引起的甲状腺功能减退症的长期替代治疗,也可用于单纯性甲状腺肿,慢性淋巴性甲状腺炎,甲状腺癌手术后的抑制(及替代)治疗,也可用于诊断甲状腺功能亢进的抑制试验。 用法用量
口服给药: 成人 (1)一般最初每日用25~50μg,最大量不超过100μg,可每隔2~4周增加25~50μg,直至维持正常代谢为止。一般维持剂量为50~200μg/日。 (2)老年或有心血管疾病患者:起始量以12.5~25μg为宜,可每3~4周增加一次剂量,每次增加12.5~25μg。用药后应密切观察患者有否心率加快,心律失常、血压改变并定期监测血中甲状腺激素水平,必要时暂缓加量或减少用量。 儿童 新生儿和儿童甲状腺功能降低或克汀病,剂量 参考下表: 建议使用剂量(每天)或遵医嘱: 年龄 - 每日量(μg) - 按体重(μg/kg) 0~6个月 - 25~50微克 - (8~10微克/千克体重) 7~12个月 - 50~70微克 - (6~8微克/千克体重) 2~5周岁 - 75~100微克 - (5~6微克/千克体重) 6~12周岁 - 100~150微克 - (4~5微克/千克体重) 12岁以上 - 150~200微克 - (2~3微克/千克体重) 用药后2~4周增加一个剂量(12.5μg~25μg),至临床表现及甲状腺激素水平完全正常。 任何疑问,请遵医嘱! 不良反应
剂量过度的表现有心绞痛、心律失常、心悸、腹泻、呕吐、震颤、兴奋、头痛、不安、失眠、多汗、潮红、体重减轻、骨骼肌痉挛等,通常在减少用量或停药数日后,上述表现消失。 禁忌
患有非甲状腺功能低下性心衰,快速型心律失常和近期出现心肌梗死者禁用,对本药过敏者禁用。 注意事项
1.老年,有心血管疾病患者,有心肌缺血或糖尿病者慎重用药; 2.有垂体功能减低或肾上腺皮质功能减退者,如需补充甲状腺制剂,在给左甲状腺素钠以前数日应先用肾上腺皮质激素。 孕妇及哺乳期妇女用药
在甲状腺替代治疗期间,必须严密监护,避免造成过低或过高的甲状腺功能,以免对胎儿及婴儿造成不良影响。微量的甲状腺激素可从乳汁中排中。 药物相互作用
1.左甲状腺素钠会增加抗凝剂作用; 2.左甲状腺素钠会升高血中苯妥英钠水平; 3.抗惊厥药如卡马西平和苯妥英钠加快左甲状腺素钠代谢,可将甲状腺素从血浆蛋白中置换出来; 4.本品与强心甙一起使用,须相应调整强心甙用量; 5.左甲状腺素钠也会增加拟交感性药物的作用; 6.左甲状腺素钠增加儿茶酚胺受体敏感性,因此会增强三环抗抑郁药的作用; 7.消胆胺减少左甲状腺素钠吸收,同时用口服避孕药,需增加本品用量。

Be sure to tell your doctor of any allergies you have whether it is to medication or food, preservatives, or dyes. Also tell your doctor of any other medications you are taking, including vitamins and supplements, and any medical conditions you may have.
It is very important that you take this medicine exactly as your doctor tells you to. Do not take more or less medicine than directed, and don’t stop taking Synthroid unless you’ve discussed it with your doctor first.
If you miss a dose of this medicine, take it as soon as you remember, unless it is almost time for your next dose. Do NOT double doses.
Check with your doctor as soon as possible if any of the following (indications of an overdose or allergic reaction) occur: headache in children; skin rash or hives; chest pain; confusion; fast or irregular heartbeat; mood swings; muscle weakness; psychosis; restlessness; yellow eyes or skin; or shortness of breath.
Levothyroxine sodium tablets and injection contain synthetic crystalline L-3,3¢,5, 5¢-tetraiodothyronine sodium salt [levothyroxine (T4) sodium]. Synthetic T4 is identical to that produced in the human thyroid gland. Levothyroxine (T4) Sodium has an empirical formula of C15H10I4NNaO4xH2O, and molecular weight of 798.86 (anhydrous). Synthroid Tablets Inactive Ingredients: Acacia, confectioner's sugar (contains cornstarch), lactose, magnesium stearate, povidone, and talc.
