药物功效：本药可用于各种原因引起的急性心力衰竭及慢性难治性心力衰竭的短期治疗。
作用：增強心肌收縮力及擴張血管的作用，使心輸出量增加，但是並不會增加心跳速率及心肌細胞的耗氧量
動力學：增加鈣離子流入肌漿膜的濃度，使心肌收縮力增強；經由cAMP的調控增加鈣攝入，減低血管平滑肌細胞因鈣離子造成的收縮，達到血管擴張的效果.
全身性血管阻力下降及肺動脈楔壓的下降.
時間：50mcg/kg IV (over 10 mins).
起始：1-3 hr 50mcg/kg IV (over 10 mins).
持續：3-5 hr. 0.59-1.13 mg/kg/day (0.375-0.75 mg/kg/min as continuous IV infusion).
劑量：建議的每日藥物使用極量是1.13 mg/公斤體重.loading dose: 50ug/kg (~ 3ml, >10min IV)maintenance dose: 0.5ug/kg/min (18ml/hr), 1 amp + D5W or N/S 90ml(0.1mg/ml, 3ml/hr ,10ug/min)
副作用：頭痛(2.9%)、低血壓(2.9%)、上心室性心率不整(3.8%)；加重心室性心率不整(12.1%)是因為縮短房室結的傳導時間。心絞痛/胸痛(1.2%)及血小板減少症

GENERIC NAME: MILRINONE LACTATE - INJECTION (MILL-rih-nohn)
BRAND NAME(S): Primacor
USES: This medication is for the short-term treatment of congestive heart failure (CHF).
HOW TO USE: This medication is given by injection into a vein. You will be monitored closely by means of regular doctor visits and routine laboratory tests to ensure the effectiveness and safety of this medication.
SIDE EFFECTS: This medication is generally well tolerated. Headache, chest pain and a slight fever have been reported as a result of this medication. If these effects persist or worsen, inform your doctor promptly. Contact your doctor immediately if you experience: irregular heartbeat. An allergic reaction to this drug is unlikely,
but seek immediate medical attention if it occurs.
Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing.
If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: This medication should not be used if you have certain medical conditions.
Before using this medicine, consult your doctor or pharmacist if you have: heart valve disease (e.g., aortic or pulmonic valvular disease). Before using this medication, tell your doctor or pharmacist your medical history, especially of: irregular heartbeat, mineral imbalance (low potassium blood level), certain other serious heart disease (e.g., hypertrophic subaortic stenosis), kidney disease, any allergies.
This medication should be used only if clearly needed during pregnancy. Discuss the risks and benefits with your doctor.
It is not known if this medication is found in breast milk. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Tell your doctor of any over-the-counter or prescription medication you may take. Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE:
If overdose is suspected, contact your local poison control center or emergency room immediately. US residents can call the US national poison hotline at 1-800-222-1222. Canadian residents should call their local poison control center directly. Symptoms of overdose may include severe dizziness.
NOTES: An oral form of this medication has been under study but does not show as great a benefit as does the injectable form.
MISSED DOSE:
If you miss a dose, take it as soon as remembered; do not take it if it is near the time for the next dose, instead, skip the missed dose and resume your usual dosing schedule. Do not "double-up" the dose to catch up.
STORAGE: Store as directed away from heat and light.
MEDICAL ALERT: Your condition can cause complications in a medical emergency.
