药品介绍：
艾可拓是唯一被大型前瞻性双盲对照研究证实具有心血管长期获益及安全性的口服糖尿病药物，并能改善胰岛素抵抗，实现持久降糖，低血糖发生少；艾可拓可同时实现持久降糖与减少心血管事件，是单药血糖控制不佳患者联合用药的优选。
药理作用
本品为噻唑烷二酮类口服抗糖尿病药物，为高选择性过氧化物酶增殖体激活受体γ(PPARγ)的激动剂，属胰岛素增敏剂。作用机制与胰岛素的存在有关，可减少外周组织和肝脏的胰岛素抵抗，增加依赖胰岛素的葡萄糖的处理，并减少肝糖的输出。与磺酰脲类不同，本品不是胰岛素促分泌药，本品是通过提高外周和肝脏的胰岛素敏感性来控制血糖水平。本品能明显增加外周组织对葡萄糖的摄取，同时使血浆胰岛素水平下降，不增加体重，有利于血胆固醇和三酰甘油趋于正常。
适应证
用于2型糖尿病(或非胰岛素依赖性糖尿病，NIDDM)。
用法用量
起始剂量为每日15mg或30mg，最大剂量为45mg，在早餐前服用，如漏服1次，第2天不可用双倍剂量。

室温密闭干燥保存，避免受潮

ACTOS(pioglitazone hydrochloride) Tablets
Actos (generic name: Pioglitazone HCl) is an once-a-daily oral drug for diabetes management. In patients with diabetes, the liver does not produce enough insulin, and therefore synthetic insulins are prescribed to regulate blood sugar levels and prevent a spiking in blood sugar. Actos is an insulin resistance reducer. It helps to regulate the blood sugar levels of diabetics, and works by regulating the levels of insulin produced in the body after eating. Actos is prescribed for type 2 diabetes only, as it is not effective in type 1 diabetes. Actos is commonly used in conjunction with other diabetes medications as part of a treatment program. Patients with diabetes commonly experience symptoms such as blurred vision, cuts/sores which take a long time to heal, itchy skin, yeast infections, leg pain, dry mouth and excessive thirst. Diabetes can be diagnosed by a simple blood test, and if you are diagnosed your doctor will discuss the most effective treatment for your particular condition.
ACTOS Rx

Indications for ACTOS:
Adjunct to diet and exercise in type 2 diabetes, as monotherapy or in combination with metformin, insulin, or a sulfonylurea.
Adult Dose for ACTOS:
≥18yrs: initially 15mg or 30mg once daily; max 45mg once daily. Reduce insulin or sulfonylurea dose if needed.
Children's Dose for ACTOS:
<18yrs: not recommended.
Pharmacological Class:
Thiazolidinedione.
Contraindications:
NYHA Class III or IV heart failure.
Warnings/Precautions:
Not for treating type 1 diabetes or diabetic ketoacidosis. Symptomatic HF: not recommended. CHF: if NYHA Class II: start at lowest approved dose and titrate carefully. Edema. May increase plasma volume (monitor for signs/symptoms of heart failure); discontinue if cardiac status deteriorates. Hepatic disease. Do not start therapy in active liver disease or if ALT >2.5xULN. Monitor ALT at baseline, then periodically. If ALT 1–2.5xULN, follow-up and monitor closely; discontinue if ALT >3xULN persists or if jaundice occurs. Not for use in patients with history of troglitazone-associated jaundice. Resumption of premenopausal ovulation in anovulatory patients may occur (may result in unintended pregnancy). Pregnancy (Cat.C): consider using insulin instead. Nursing mothers: not recommended.
Interactions:
Potentiated by CYP2C8 inhibitors (eg, gemfibrozil). Antagonized by CYP2C8 inducers (eg, rifampin). Antagonizes oral contraceptives, midazolam. Monitor glycemic control more frequently with ketoconazole. Monitor for heart failure with insulin.
Adverse Reactions:
Upper respiratory tract infection, headache, sinusitis, pharyngitis, myalgia, anemia, edema, weight gain, hypoglycemia, dyspnea; changes in serum lipids (may be beneficial); also women: risk of fracture.

適應症  
用於治療第2型糖尿病（即非胰島素依賴型糖尿病或NIDDM成年型糖尿病）。
用法/用量  
本藥限由醫師使用。
本藥每日服用一次，於飯前或飯後服用均可，糖尿病的治療應視個別病患而異。
形 狀  
Pioglitazone Hydrochloride是無臭的白色結晶狀粉末；為圓形扁平錠。  
儲 法  
保存於攝氏25°C（華氏77°F）的室溫下，可允許短時間放於攝氏15～30°C（華氏59～86°F）環境中。保持鋁箔包裝完整，並且注意避免水氣和潮濕。

15mg錠劑：白色至黃白色，錠劑一面有一字刻痕標示圖樣及390字樣，另一面標示有十字刻痕及15字樣。
