【化学成分】本品化学名称：3-［(1-乙氧羰基-3-苯基-（1S）-丙基)氨基］2，3，4，5-四氢-2-氧代-1H-1-（3S）苯并氮杂卓-1-乙酸盐酸盐。
【功效主治】
1.高血压（可单独应用或与其他降压药如利尿药合用）2.心功能不全（可单独应用或与强心药利尿药同用）
【药理作用】
1 药理：
（1）降压：本品在肝内水解为苯那普利拉，成为一种竞争性的血管紧张素转换酶抑制剂，阻止血管紧张素I转换为血管紧张素II，使血管阻力降低，醛固酮分泌减少，血浆肾素活性增高。苯那普利拉还抑制缓激肽的降解，也使血管阻力降低，产生降压作用。
（2）减低心脏负荷：本品扩张动脉与静脉，降低周围血管阻力或心脏后负荷，降低肺毛细血管嵌压或心脏前负荷，也降低肺血管阻力，从而改善心排血量，使运动耐量和时间延长。
2 毒理:大鼠和小鼠持续口服苯那普利2年，剂量为每天150mg/kg，未发现本品有致癌性。（该剂量按mg/kg计算，为人类最大用量的110倍；按mg/m2计算，为人类最大用量的18倍和9倍）。不论在细菌试验中，还是在体外培养的哺乳动物细胞试验中均未发现本品有致突变性。雌、雄大鼠口服苯那普利，剂量为每天50～150mg/kg，未发现本品影响生殖能力。（该剂量按mg/kg计算，为人类最大用量的37～375倍；按mg/m2计算，为人类最大用量的6～60倍）。
【药物相互作用】
1 与利尿药合用降压作用增强，可能引起严重低血压。故原用利尿药应停药或减量，本品开始用小剂量，逐渐调整剂量。
2 与其他扩血管药合用可能导致低血压。如合用，应从小剂量开始。
3 与潴钾利尿药（如螺内酯、氨苯喋啶、阿米洛利）合用可引起血钾过高。
4 与非甾体类抗炎止痛药合用可通过抑制前列腺素合成及水钠潴留，使本品降压作用减弱。
5 与其他降压药合用，降压作用加强。其中与引起肾素释出或影响交感活性的药物呈较大的相加作用，与β受体阻滞剂合用呈小于相加的作用。
【不良反应】
1 常见的有：头痛、头晕、疲乏、嗜睡、恶心、咳嗽。最常见的为头痛和咳嗽。
2 少见的有：症状性低血压、体位性低血压、晕厥、心悸、周围性水肿、皮疹、皮炎、便秘、胃炎、焦虑、失眠、感觉异常、关节痛、肌痛、哮喘等。血管神经性水肿罕见。
【禁忌症】
1 对苯那普利或其他血管紧张素转换酶抑制剂过敏者。
2 有血管神经性水肿史者。
3 孤立肾、移植肾、双侧肾动脉狭窄而肾功能减退者。

【用法用量】
药理

【贮藏方法】密封，30℃以下保存。

【注意事项】
1 血管神经性水肿：服用本品曾发生过唇或面部水肿，如出现该症状，应立即停药，监护患者，直到水肿消失。声门、舌、喉部水肿可能引起气道阻塞，应停药，并立即进行适当治疗，如：皮下注射1：1000肾上腺素溶液（0.3ml～0.5ml）。
2 低血压：严重缺钠的血容量不足者服用本品时可能发生低血压（如接受大量利尿药或透析治疗者）。开始服用本品前数天应停用利尿药或采取其他措施补充体液。对有可能发生严重低血压者（如心功能不全病人），服用首剂后应严密监护，直到血压稳定。如果发生低血压，应采取卧位，必要时静脉滴注生理盐水。
3 粒细胞减少：自身免疫性疾病及肾功能不全者出现白细胞或粒细胞减少机会增多。对肾功能不全或有白细胞减少者，最初3个月内每2周检查白细胞计数及分类1次，以后定期检查。
4 肾功能不全：少数患者服用本品后可出现暂时性血尿素氮、肌酐升高，停用本品和/或利尿药，即可恢复。对肾功能不全者，在治疗前几周要密切监测肾功能，以后应定期检查肾功能。用本品时如肌酐清除率<30ml/min或血尿素氮、肌酐升高，须减低本品的剂量和/或停用利尿药。
5 其他：偶见血钾升高，尤其在肾功能不全和并用治疗低血钾的药物时。偶见氨基转移酶升高。脑或冠状动脉供血不足，可因血压降低而加重。肝功能障碍时本品在肝内的代谢降低。

Lotensin is one of a class of drugs known as ACE Inhibitors. When used as an antihypertensive, Lotensin helps lower high blood pressure. As a vasodilator, Lotensin may be used to treat congestive heart failure.

Lotensin Description
This medicine is an ACE inhibitor used to treat high blood pressure. It may also be used to treat other conditions as determined by your doctor.

Lotensin Directions
Follow the directions for using this medicine provided by your doctor. This medicine may be taken on an empty stomach or with food. STORE THIS MEDICINE at room temperature below 86 degrees F (30 degrees C) in a tightly-closed container away from heat, moisture, and light. Take this medicine regularly to receive the most benefit from it. Taking this medicine at the same times each day will help you to remember. CONTINUE TO TAKE THIS MEDICINE even if you feel well. Do not miss any doses. IF YOU MISS A DOSE OF THIS MEDICINE, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. DO NOT take 2 doses at once.

Lotensin Cautions
DO NOT TAKE THIS MEDICINE if you have had an allergic reaction to it, to other ACE inhibitors, or if you are allergic to any ingredient in this product. DO NOT EXCEED THE RECOMMENDED DOSE without checking with your doctor. IT MAY TAKE 1 TO 2 WEEKS for this medicine to work. Do not stop using this medicine without checking with your doctor. Laboratory and/or medical tests including liver function, kidney function, blood pressure, blood counts, and blood electrolytes may be performed to monitor your progress or to check for side effects. KEEP ALL DOCTOR AND LABORATORY APPOINTMENTS while you are taking this medicine. BEFORE YOU HAVE ANY MEDICAL OR DENTAL TREATMENTS, EMERGENCY CARE, OR SURGERY, tell the doctor or dentist that you are using this medicine. THIS MEDICINE MAY CAUSE dizziness, lightheadedness, or fainting, especially when you begin taking this medicine. DO NOT DRIVE, OPERATE MACHINERY, OR DO ANYTHING ELSE THAT COULD BE DANGEROUS until you know how you react to this medicine. Using this medicine alone, with other medicines, or with alcohol may lessen your ability to drive or to perform other potentially dangerous tasks. If dizziness occurs, sit up or stand slowly. THIS MEDICINE MAY CAUSE increased sensitivity to the sun. Avoid exposure to the sun, sunlamps, or tanning booths until you know how you react to this medicine. Use a sunscreen or protective clothing if you must be outside for a prolonged period. DRINK PLENTY OF FLUIDS during exercise or other activities where excessive sweating may occur. IF YOU EXPERIENCE difficulty breathing; tightness of chest; swelling of eyelids, face, or lips; or if you develop a rash or hives, tell your doctor immediately. Do not take any more of this medicine unless your doctor tells you to do so. THIS MEDICINE MAY LOWER YOUR RESISTANCE TO INFECTION. Prevent infection by avoiding contact with people with colds or other infections. Do not touch your eyes or the inside of your nose unless you have thoroughly washed your hands first. DO NOT USE A SALT SUBSTITUTE or other potassium supplement without checking with your doctor. BEFORE YOU BEGIN TAKING ANY NEW MEDICINE, either prescription or over-the-counter, check with your doctor or pharmacist. CAUTION IS ADVISED WHEN USING THIS MEDICINE IN THE ELDERLY because they may be more sensitive to the effects of the medicine. FOR WOMEN: USE OF THIS MEDICINE DURING PREGNANCY has resulted in fetal and newborn death. If you think you may be pregnant, contact your doctor immediately. THIS MEDICINE IS EXCRETED in breast milk. DO NOT BREAST-FEED while taking this medicine. IF YOU HAVE DIABETES, this medicine may affect your blood sugar. Check blood sugar levels closely and ask your doctor before adjusting the dose of your diabetes medicine.

Lotensin Side Effects
SIDE EFFECTS, that may occur while taking this medicine include headache, dizziness or lightheadedness, drowsiness, and cough. If they continue or are bothersome, check with your doctor. CHECK WITH YOUR DOCTOR AS SOON AS POSSIBLE if you experience severe lightheadedness or fainting; fast or irregular heartbeat; prolonged nausea, vomiting, or diarrhea; unusual joint pain; or unusual muscle pain, cramps, or weakness. CONTACT YOUR DOCTOR IMMEDIATELY if you experience swelling of hands, legs, face, lips, eyes, throat, tongue, or trunk; difficulty swallowing or breathing; hoarseness; chest pain; unusual stomach pain; yellowing of the skin or eyes; darkening of your urine; sore throat; or fever. AN ALLERGIC REACTION to this medicine is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include rash, itching, swelling, severe dizziness, or trouble breathing. If you notice other effects not listed above, contact your doctor, nurse, or pharmacist.
