英文药名: Primacor(Milrinone Injection) 中文药名: 乳酸米力农注射液 药品介绍 【中文别名】米力农、甲氰吡酮、甲腈联吡啶酮、卡巴喹伦、米利酮 【英文别名】Corotrop、Corotrope、Milrinone Lactate、Primacor 【药品类别】强心药 【药理作用】 本品系氨利酮衍生物，具有正性肌力和扩张血管作用。主要通过抑制磷酸二酯酶同功酶 Ⅲ，使心肌内环磷酸腺苷(cAMP)含量升高、加快Ca2+内流而增强心肌收缩。其正性肌力作用较氨利酮强 10～40倍。其扩张外周血管作用也较强。其心血管效应与剂量大小有关。小剂量时主要表现为正性肌力作用，当剂量增大，逐渐达到稳定状态的最大正性肌力效应时，其扩张血管的作用也随着剂量的增加而逐渐加强。 【药动学】 口服吸收良好，给药后血浆药物峰值浓度与给药剂量有关。一般在口服后 0.5h开始生效，1～3h达峰值，持续时间为 4～6h。生物利用度为 92％，心衰患者的消除半减期为 3.3～2.7h。主要在肝脏代谢失活，80％以原形从尿中排出。 【适应症】 临床口服或静滴对慢性充血性心力衰竭均有满意疗效，其增加心脏指数优于氨吡酮，对动脉压和心率无明显影响． 主要用于治疗重度充血性心力衰竭患者，也可在常规应用洋地黄和利尿剂的基础上加用本品治疗。本品短期奏效明显，且副作用较氨利酮为小，但其远期疗效尚待进行大量的临床观察而定论。 适用于强心药、利尿药、血管扩张药治疗无效的充血性心力衰竭。 【用法用量】 米力农注射液可用适量生理盐水或葡萄糖液稀释后，先按37.5ug/kg-50ug/kg缓慢静脉注射10分钟(注射过快，可能出现室性早搏)，继续以0.37ug/kg-0.75ug/kg/min 静脉滴注，此后根据临床效应调整用量。 【贮藏】密闭保存 【不良反应】 剂量过大可有低血压(血压下降),心动过速.长期使用可增加慢性心衰患者死亡率. 较少见，可有低血压、头痛、胸痛、肌无力、失眠、震颤、血小板减少、低血钾和室性心律失常。 米力农治疗时有12％的病人出现室性心律紊乱，所以在治疗充血性心力衰竭时应密切注意这种不良反应的出现。据报道，发生的室性心律紊乱中有9％为心室异位搏动，3％为非持续性室性心律紊乱，1％的为持续性室性心律紊乱和0.2％的室性纤维颤动。其它的不良反应有室上性心律紊乱(4％)、低血压(3％)、心绞痛或胸痛(1％)和头痛(3％)、震颤和血小板减少症。 【禁忌症】 对本品过敏者及严重室性心律失常禁用。 急性心肌梗死早期患者及孕妇慎用。 对严重主动脉梗阻或肺动脉瓣病应小心使用，更不能代替这些梗阻的外科手术。 肝、肾功能损害严重者慎用。 尚无用于孕妇哺乳及儿童的经验，使用时需权衡利弊。 【药物相互作用】 本品与地高辛合用，有协同的强心作用。 呋塞米和米力农两者在溶液中混合时可产生沉淀，因此两者不能混在一起同时注射。也不可与布美他尼配伍。 与常用强心药、利尿药、血管扩张药合用，临床未见不良相互作用，与多巴胺、多巴酚丁胺合用有协同作用。 GENERIC NAME: MILRINONE LACTATE - INJECTION (MILL-rih-nohn) USES: This medication is for the short-term treatment of congestive heart failure (CHF). HOW TO USE: This medication is given by injection into a vein. You will be monitored closely by means of regular doctor visits and routine laboratory tests to ensure the effectiveness and safety of this medication. SIDE EFFECTS: This medication is generally well tolerated. Headache, chest pain and a slight fever have been reported as a result of this medication. If these effects persist or worsen, inform your doctor promptly. Contact your doctor immediately if you experience: irregular heartbeat. An allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist. PRECAUTIONS: This medication should not be used if you have certain medical conditions. Before using this medicine, consult your doctor or pharmacist if you have: heart valve disease (e.g., aortic or pulmonic valvular disease). Before using this medication, tell your doctor or pharmacist your medical history, especially of: irregular heartbeat, mineral imbalance (low potassium blood level), certain other serious heart disease (e.g., hypertrophic subaortic stenosis), kidney disease, any allergies. This medication should be used only if clearly needed during pregnancy. Discuss the risks and benefits with your doctor. It is not known if this medication is found in breast milk. Consult your doctor before breast-feeding. DRUG INTERACTIONS: Tell your doctor of any over-the-counter or prescription medication you may take. Do not start or stop any medicine without doctor or pharmacist approval. OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. US residents can call the US national poison hotline at 1-800-222-1222. Canadian residents should call their local poison control center directly. Symptoms of overdose may include severe dizziness. NOTES: An oral form of this medication has been under study but does not show as great a benefit as does the injectable form. MISSED DOSE: If you miss a dose, take it as soon as remembered; do not take it if it is near the time for the next dose, instead, skip the missed dose and resume your usual dosing schedule.
