部分中文安碘达隆信息资料(仅供参考) 英文名称:Amiodarone 中文别名:安碘达隆、安律酮、胺碘达龙、乙胺碘呋酮、乙碘酮 英文别名:Amiodar、Atlansil、Atlansil Cordarone、Cardilol、Corbionar、Leurquin、Rythmarone、Sedacoron、Trangorex、Tranguarone 适应症 临床适用于室性和室上性心动过速和早搏、阵发性心房扑动和颤动、预激综合征等。 也可用于伴有充血性心力衰竭和急性心肌梗塞的心律失常。对其他βˉ受体阻断剂无效的顽固性阵发性心动过速也能奏效。另外,也用于慢性冠状动脉功能不全和心绞痛的治疗。 1.房性心律失常(心房颤动或心房扑动的转律,以及转律后窦性心律的维持。 2.结性心律失常 3.室性心律失常(有生命危险的室性期前收缩,室性心动过速,室性心动过速或心室纤颤的预防。 4.伴有预激综合症的心律失常。综观其药理特性,可达龙可适用于上述心律失常,尤其是伴随器质性心脏疾病而出现的心律失常。 用量用法 口服:开始每次mg,1日3次,饭后服;3日后改用维持量,每次mg,1日~2次,或每次mg,1日3次。 静滴:300mg加入250ml等渗盐水中,30分钟内滴完。 负荷量600mg/日,连续8-10日;维持量:根据个体反应采用最小有效量一般为100-400mg/日,也可采用每周停药2天的间断治疗,即200mg/日,每周用5天。 不良反应 (1)心血管:较其他抗心律失常药对心血管的不良反应要少。 ①窦性心动过缓、窦性停搏或窦房阻滞,阿托品不能对抗此反应; ②房室传导阻滞; ③偶有Q-T间期延长伴扭转性室性心动过速;主要见于低血钾和并用其它延长QT的药物时; ④以上不良反应主要见于长期大剂量和伴有低血钾时,以上情况均应停药,可用升压药、异丙肾上腺素、碳酸氢钠(或乳酸钠)或起搏器治疗;注意纠正电解质紊乱;扭转性室性心动过速发展成室颤时可用直流电转复。由于本品半衰期长,故治疗不良反应需持续5~10天。 (2)甲状腺: ①甲状腺机能亢进,可发生在用药期间或停药后,除突眼征以外可出现典型的甲亢征象,也可出现新的心律失常,化验T3、T4均增高,TSH下降。发病率约2%,停药数周至数月可完全消失,少数需用抗甲状腺药、普萘洛尔或肾上腺皮质激素治疗; ②甲状腺机能低下,发生率1%~4%,老年人较多见,可出现典型的甲状腺机能低下征象,化验TSH增高,停药后数月可消退,但粘液性水肿可遗留不消,必要时可用甲状腺素治疗。 (3)胃肠道:便秘,少数人有恶心、呕吐、食欲下降,负荷量时明显。 (4)眼部:服药3个月以上者在角膜中基底层下1/3有黄棕色色素沉着,与疗程及剂量有关,儿童发生较少。这种沉着物偶可影响视力,但无永久性损害。少数人可有光晕,极少因眼部副作用停药。 (5)神经系统:不多见,与剂量及疗程有关,可出现震颤、共济失调、近端肌无力、锥体外体征,服药1年以上者可有周围神经病,经减药或停药后渐消退。 (6)皮肤:光敏感与疗程及剂量有关,皮肤石板蓝样色素沉着,停药后经较长时间(1~2年)才渐退。其他过敏性皮疹,停药后消退较快。 (7)肝脏:肝炎或脂肪浸润,氨基转移酶增高,与疗程及剂量有关。 (8)肺脏:肺部不良反应多发生在长期大量服药者(一日~1.2g)。主要产生过敏性肺炎,肺间质或肺泡纤维性肺炎,肺泡及间质有泡沫样巨噬细胞及2型肺细胞增生,并有纤维化,小支气管腔闭塞。临床表现有气短、干咳及胸痛等,限制性肺功能改变,血沉增快及血液白细胞增高,严重者可致死。需停药并用肾上腺皮质激素治疗。 (9)其他:偶可发生低血钙及血清肌酐升高。 禁 忌 ⑴严重窦房结功能异常者禁用; ⑵II或III度房室传导阻滞者禁用; ⑶心动过缓引起晕厥者禁用; ⑷对本品过敏者禁用。 注意事项 (1)过敏反应,对碘过敏者对本品可能过敏。 (2)对诊断的干扰: ①心电图变化:例如P-R及Q-T间期延长,服药后多数患者有T波减低伴增宽及双向,出现u波,此并非停药指征; ②极少数有AST、ALT及碱性磷酸酶增高; ③甲状腺功能变化,本品抑制周围T4转化为T3,导致T4及rT3增高和血清T3轻度下降,甲状腺功能检查通常不正常,但临床并无甲状腺功能障碍。甲状腺功能检查不正常可持续至停药后数周或数月。 (3)下列情况应慎用: ①窦性心动过缓; ②Q-T延长综合征; ③低血压; ④肝功能不全; ⑤肺功能不全; ⑥严重充血性心力衰竭。 (4)多数不良反应剂量有关,故需长期服药者尽可能用最小有效维持量,并应定期随诊,用药期间应注意随访检查: ①血压; ②心电图,口服时应特别注意Q-T间期; ③肝功能; ④甲状腺功能,包括T3、T4及促甲状腺激素,每3~6个月1次; ⑤肺功能、肺部X射线片,每6~12个月1次; ⑥眼科检查。 (5)本品口服作用的发生及消除均缓慢,临床应用根据病情而异。对危及生命的心律失常宜用短期较大负荷量,必要时静脉负荷。而对于非致命性心律失常,应用小量缓慢负荷; (6)本品半衰期长,故停药后换用其他抗心律失常药时应注意相互作用。 孕妇及哺乳期妇女用药 本品可以通过胎盘进入胎儿体内,大鼠实验已证实胺碘酮对胎儿有毒性作用临床上有孕妇服用胺碘酮引起胎儿先天性甲状腺肿、甲亢和甲低的报道。新生儿血中原药及代谢产物为母体血浓度的25%。已知碘也可通过胎盘,故孕妇使用时应权衡利弊。本品及代谢物可从乳汁中分泌,服本品者不宜哺乳。 儿童用药 儿童中应用胺碘酮的安全性和有效性尚不明确。 老年用药 老年人口服胺碘酮需严密监测心电图、肺功能.

Cordarone can interact with grapefruit and grapefruit juice, and the interaction may have dangerous effects. You should discuss the use of grapefruit and grapefruit juice with your doctor. •It is important to take Cordarone regularly to get the most benefit. •Do not skip doses or change the dosing schedule without talking to your doctor. Missed or delayed doses could be harmful. •Your doctor may want you to have blood tests or other medical evaluations during treatment with Cordarone to monitor progress and side effects. •Store Cordarone at room temperature away from moisture and heat. Precautions •Do not consume grapefruit or grapefruit juice during treatment with Cordarone. Cordarone can interact with grapefruit and grapefruit juice, and the interaction may have dangerous effects. You should discuss the use of grapefruit and grapefruit juice with your doctor. •Use caution when driving, operating machinery, or performing other hazardous activities. Cordarone may cause dizziness, drowsiness, or blurred vision. If you experience dizziness, drowsiness, or blurred vision, avoid these activities. •Avoid prolonged exposure to sunlight. Cordarone may cause the skin to turn a bluish-gray color. This reaction is worse with exposure to the sun. Use a sunscreen and wear protective clothing when exposure to the sun is unavoidable. •Follow any other special instructions given by your doctor regarding food, beverages, or activities (e.g., driving). Side Effects Of Cardarone (Amiodarone) •If you experience any of the following serious side effects, stop taking Cordarone and seek emergency medical attention: ◦an allergic reaction (difficulty breathing; closing of the throat; swelling of the lips, tongue, or face; or hives); ◦a new or a worsening irregular heartbeat pattern; ◦wheezing, coughing, chest pain, or chest discomfort; ◦yellowing of the skin or eyes; ◦changes in vision, blurred vision, or decreased vision; ◦cough or shortness of breath; ◦fainting; ◦tremor or involuntary movements; or ◦skin rash; •Other, less serious side effects may also occur. Continue to take Cordarone and talk to your doctor if you experience ◦dizziness or tiredness; ◦upset stomach, vomiting, decreased appetite, or diarrhea; ◦changes in taste; ◦poor coordination; ◦numbness, or tingling; or ◦bluish-gray discoloration of the skin. •Side effects other than those listed here may also occur. Talk to your doctor about any side effect that seems unusual or that is especially bothersome. amiodarone Hcl Amiodarone - Injection Amiodarone Hydrochloride Injection How To Use Amiodarone Hcl is given by injection into a vein (IV) as directed by your doctor. Follow all instructions for proper mixing and dilution with the correct IV fluids. If you have questions regarding the use of Amiodarone Hcl, consult your pharmacist. Before using, check this product visually for particles or discoloration. If either is present, do not use the liquid. Do not exceed the recommended rate of infusion. Infusing amiodarone too fast may result in serious side effects such as severe lowering of the blood pressure. Your dosage and infusion rate may be adjusted depending upon your medical condition and response to therapy. A chemical (DEHP) found in plastic IV tubing may harm young children (male reproductive system effects). Amiodarone can cause this chemical to leach out of the tubing. Therefore, caution is advised when using Amiodarone Hcl in children. Special administration guidelines should be followed (e.g., multiple bolus dosing) to minimize exposure to DEHP. Consult the pharmacist for more information. Side Effects Also see the Warning section. Diarrhea, nausea, vomiting, or dizziness may occur. If any of these effects persist or worsen, notify your doctor promptly. Amiodarone Hcl, when used in combination with simvastatin, may infrequently cause muscle damage (which can rarely lead to a very serious condition called rhabdomyolysis). Tell your doctor immediately if you develop: muscle pain/tenderness/weakness (especially with fever or unusual tiredness). Tell your doctor immediately if any of these unlikely but serious side effects occur: severe dizziness, slowed heartbeat, uncontrolled shaking/tremor, unexplained weight gain, swelling of the hands or feet, irregular pulse, vision changes (e.g., seeing halos, blurred vision, loss of vision - See Warnings), easy bruising/bleeding, persistent sore throat. Amiodarone Hcl may cause skin reactions similar to a serious sunburn. Infrequently with long-term amiodarone treatment, you may develop blue-gray discoloring of the skin, particularly of the face and hands. This effect is not harmful and usually reverses, sometimes incompletely, after Amiodarone Hcl is stopped. Avoiding prolonged sun exposure may help to prevent this effect. See also Precautions. In the unlikely event you have an allergic reaction to Amiodarone Hcl, seek immediate medical attention. Symptoms of an allergic reaction include: rash, itching, swelling, severe dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist. Precautions Amiodarone Hcl should not be used if you have certain medical conditions. Before using Amiodarone Hcl, consult your doctor or pharmacist if you have: cardiogenic shock, certain types of irregular heartbeats or heart rhythms (e.g., severe sinus node dysfunction resulting in marked sinus bradycardia, second or third degree atrioventricular block, bradycardia with associated syncope except when a pacemaker is used). Before using Amiodarone Hcl, tell your doctor or pharmacist your medical history, especially of: liver disease, heart problems (e.g., congestive heart failure, low blood pressure), lung diseases, thyroid problems, upcoming major surgery, electrolyte problems such as low potassium or low magnesium, any allergies. Amiodarone Hcl may make you dizzy; use caution engaging in activities requiring alertness such as driving or using machinery. Limit alcoholic beverages. Amiodarone Hcl may make you more sensitive to the sun. Avoid prolonged sun exposure. Use a sunscreen and wear protective clothing when outdoors. Caution is advised when using Amiodarone Hcl in the elderly because they may be more sensitive to the effects of Amiodarone Hcl, especially slowing of the heartbeat. A preservative (benzyl alcohol) which may be found in this product or in the liquid used to mix this product (diluent) can infrequently cause serious problems (sometimes death) if given by injection to an infant during the first months of life (neonatal period). The risk is greater with lower birth weight infants and is greater with increased amounts of benzyl alcohol. Symptoms include sudden gasping, low blood pressure, or a very slow heartbeat. Report these symptoms to the doctor immediately should they occur. If possible, a preservative-free product should be used when treating neonates. Amiodarone Hcl is not recommended for use during pregnancy due to the potential for fetal harm. Consult your doctor for more details. Amiodarone passes into breast milk. Breast-feeding is not recommended while using Amiodarone Hcl. Missed Dose If you miss a dose, contact the doctor immediately. Do not increase the rate of infusion above the recommended rate to catch up. Drug Interactions Amiodarone Hcl should not be used with the following medications because very serious interactions may occur: agalsidase beta, dofetilide, certain HIV protease inhibitors (nelfinavir, ritonavir), quinupristin/dalfopristin. If you are currently using any of these medications, tell your doctor or pharmacist before starting amiodarone. Other drugs besides amiodarone which may affect the heart rhythm (QTc prolongation in the EKG) include flecainide, disopyramide, pimozide, quinidine, sotalol, procainamide, and sparfloxacin among others. QTc prolongation can infrequently result in serious, rarely fatal, irregular heartbeats. Consult your doctor or pharmacist for details. Ask for instructions about whether you need to stop any other QTc-prolonging drugs you may be using in order to minimize the risk of this effect. Before using Amiodarone Hcl, tell your doctor or pharmacist of all prescription and nonprescription products you may use, especially of: blood thinners (e.g., warfarin), cimetidine, cyclosporine, digoxin, fentanyl, local and general anesthesia, heart medications (e.g., diltiazem, verapamil, beta-blockers such as propranolol), other HIV protease inhibitors (e.g., amprenavir, indinavir), methotrexate, phenytoin, rifampin, statins (e.g., lovastatin, simvastatin), St. John s wort.
