【药物名称】卡铂 Carboplatin 【药物类别】其他抗肿瘤药 【药物别名】碳铂、长波铂、顺二氨环丁烷铂、顺二氨环丁烷羧、酸铂  【分子式成分】 化学名： 顺式-1，1-环丁烷二羧酸二氨铂。分子式：C6H12N2O4Pt。分子量：371.26。  【药理毒理】 卡铂对小细胞肺癌、卵巢癌、睾丸肿瘤、头颈部鳞癌及恶性淋巴瘤有较好的疗效；对膀胱癌及子宫颈癌也有一定疗效。 【药 动 学】 蛋白结合率很低，卡铂中的铂与血浆蛋白的结合是不可逆的，缓慢排出体外。t1/2α为1.1h～2h，t1/2β为2.6h～5.9h。 【适 应 证】 卵巢癌、小细胞肺癌、非小细胞肺癌、头颈部鳞癌、食管癌、睾丸癌、精原细胞瘤、膀胱癌、间皮瘤、小儿脑瘤等。 【不良反应】 常见骨髓抑制、注射部位疼痛，偶见过敏反应、粘膜炎或口腔炎、周围神经毒性、耳毒性、视力模糊及恶心呕吐、便秘或腹泻、食欲减退等。 【相互作用】 1.尽量避免与可能损害肾功能的药物如氨基糖甙类抗菌素同时使用。 2.与其它抗癌药联合应用时，应注意适当降低剂量。 3.本品应避免与铝化合物接触，也不宜与其它药物混合滴注。 【用法用量】 静脉滴注，临用前用5%葡萄糖注射液制成10mg/ml的浓度，再加入5%葡萄糖注射液250ml～500ml中，每次按体表面积0.3g～0.4g/m2，1次/4周，2周期～4周期为一疗程。 第一次用药后的剂量根据白细胞、血小板计数调节： ①当中性粒细胞>2×109/L，血小板>100×109/L 时，给予前次剂量的125%； ②当中性粒细胞在0.5～2×109/L 、血小板在50～100×109/L时，按前次剂量给予； ③当中性粒细胞<0.5×109/L、血小板<50×109/L时，给予前次剂量的75％。 【注意事项】 妊娠D类，孕妇、哺乳期妇女及对含铂化合物有过敏史者禁用。水痘、带状疱疹、感染、肾功能减退、老年病人应慎用。 Carboplatin / Ebewe注射 卡铂15毫升小瓶含有150毫克 卡铂45毫升小瓶含有450毫克 CARBOPLATIN injection, solution DESCRIPTION Carboplatin Injection is supplied as a sterile, aqueous solution available in 50 mg/5 mL, 150 mg/15 mL, 450 mg/45 mL or 600 mg/60 mL multi-dose vials containing 10 mg/mL of carboplatin for administration by intravenous infusion. Each mL contains 10 mg carboplatin and Water for Injection, USP.   Carboplatin can be given as a drip (infusion) in one of the following ways: through a fine tube inserted into a vein, usually in the back of your hand (cannula) through a fine, plastic tube inserted under the skin and into a vein near your collarbone (central line) into a fine tube inserted into a vein in the crook of your arm (PICC line). The infusion takes about 60 minutes. Chemotherapy is usually given as a course of several sessions (cycles) of treatment over a few months. The length of your treatment and the number of cycles you have will depend on the type of cancer you're being treated for. Carboplatin is commonly given alongside other chemotherapy drugs as part of a combination treatment (regimen). Your nurse or doctor will discuss your treatment plan with you. Before you begin your treatment your doctor will arrange for you to have blood tests. You will usually be given anti-sickness drugs before and/or during your treatment. Possible side effects Back to top Each person’s reaction to chemotherapy is different. Some people have very few side effects while others may experience more. The side effects described here won't affect everyone who has carboplatin and may be different if you're having more than one type of chemotherapy drug. We have outlined the most common side effects but haven't included those that are rare and unlikely to affect you. If you notice any effects that are not listed here, discuss them with your doctor, chemotherapy nurse or pharmacist. Risk of infection Carboplatin can reduce the number of white blood cells, which help fight infection. White blood cells are produced by the bone marrow. If the number of your white blood cells is low you'll be more prone to infections. A low white blood cell count is called neutropenia. Neutropenia begins seven days after treatment, and your resistance to infection is usually at its lowest 10–14 days after chemotherapy. The number of your white blood cells will then increase steadily and usually return to normal before your next cycle of chemotherapy is due.
