【药理】
本品能通过细胞膜，在细胞内转变成cAMP，直接增加细胞内cAMP含量，对心脏能增强心肌收缩力使心搏出量增加，并能扩张外周血管降低血管阻力，使心脏负荷减轻，改善外周循环；在代谢方面，能动员肝糖原，同时促进被儿茶酚胺抑制的胰岛素分泌，增加的胰岛素可抑制儿茶酚胺引起的脂肪分解亢进，使游离脂肪酸减少，同时促进组织摄入糖。结果在休克时能促进能量代谢，抑制组织中高能量磷酸化物质的减少。其作用可分述如下：
(1)与cAMP不同，本品能通过细胞膜，直接在细胞内发挥作用。
(2)抑制磷酸二酯酶活性，但本身并不被磷酸二酯酶分解。
(3)激活蛋白磷酸化酶，增加Ca++移动，从而增强心肌的收缩和舒张功能，且这种增强心脏功能作用并不被β阻断药所抑制。
(4)扩张外周血管，降低血管阻力，从而改善外周循环。
(5)增加肾血流量使尿排泄量增加，发挥利尿作用。
(6)可解除休克时儿茶酚胺浓度上升对胰岛素分泌的抑制作用，使分泌亢进，由此促进组织对糖的摄取，改善能量代谢。
(7)可延长小鼠和大鼠的存活率。
本品无抗原性。
健康成人静脉内一次给药300mg，原形药物的血浓度半衰期是5.5分钟；心脏手术后的患者静脉内滴注时，原形药物的血浓度随时间而增加，且与给药速度成正比。
休克时增强心脏收缩力、降低外周血管阻力、促进胰岛素分泌、减少血浆游离脂肪酸与无机磷，并有利尿作用。
【适应症】
心脏手术后休克和严重心力衰竭患者为对象。
【禁用慎用】
慎用于急性心肌梗塞以及心肌疾病患者，心律失常患者。
孕妇和小儿慎用。
【给药说明】
给药前应对体液、呼吸等进行必要的处理。给药时应监护血压、脉率、心电图、尿量、全身状态以及肺动脉楔嵌压、心搏出量等。
本品没有外周血管收缩作用，因此，在血压过低时应考虑给予外周血管收缩剂等其它适当处理。
【不良反应】
(1)循环系统：偶见血压下降、肺动脉楔嵌压上升、期外收缩、心搏出量降低、室性心动过速、心率增加、心悸等。
(2)消化系统：偶见恶心、呕吐等。
(3)其它：偶见头痛、倦怠感、发热等。
临床评价了308例患者，其中47例(15.3％)发生副作用，主要是血压下降4.5％(14人次)，心率增加4.2％(13人次)，期外收缩增加1.3％(4人次)等循环系统症状(13.3％，41人次)和恶心1.6％(5人次)等消化系统症状(2.3％，7人次)。对血液、肝功能、肾功能等15个项目的临床检查结果未见有异常改变。

Abstract:Bucladesine sodium ointment (Actosin ointment) is a dermatological antiulcer agent which exerts pronounced effects on wound healing. However, bucladesine sodium may be decomposed to form butyryl acid under some circumstances, leading to bringing about a slight odor. To meet the requirements of medical institutions, fragrances can be added to mask the smell of butyryl acid. We have tested of bucladesine sodium ointment (New fragrance), bucladesine sodium ointment (Old fragrance), ointment base (New fragrance) and the degradation products of bucladesine sodium ointment (New fragrance) for primary skin irritation and cumulative skin irritation. In the primary and cumulative skin irritation tests, no obvious changes related to the test substance were observed, indicating that bucladesine sodium ointment and degradated bucladesine sodium ointment have no irritative effects on rabbit skin.
