【药理作用】
埃索美拉唑是奥美拉唑的S－异构体，通过特异性的靶向作用机制减少胃酸分泌，为壁细胞中质子泵的特异性抑制剂。
【吸收、分布、消除】
吸收与分布：本品吸收迅速，口服后约1~2小时血浆浓度达到高峰。绝对生物利用度为89%。
代谢与排泄：埃索美拉唑完全经细胞色素P450酶系统(CYP)代谢。一次口服剂量的近80%以代谢物形式从尿中排泄，其余的从粪便中排出。尿中的原形药物不到1%。用法用量
药片应整片吞服，而不应当咀嚼或压碎。 胃食管反流性疾病(GERD) 　　
－糜烂性反流性食管炎的治疗：40mg每日一次，连服四周。
对于食管炎未治愈或持续有症状的患者建议再服药治疗四周。 　　
－已经治愈的食管炎患者防止复发的长期维持治疗：20mg每日一次。
－胃食管反流性疾病(GERD)的症状控制：没有食管炎的患者20mg每日一次。如果用药4周症状未获控制，应对患者作进一步的检查。一旦症状消除，随后的症状控制可采用即时疗法，即需要时口服20mg，每日一次。
与适当的抗菌疗法联合用药根除幽门螺杆菌，并且愈合与幽门螺杆菌相关的十二指肠溃疡、预防与幽门螺杆菌相关的消化性溃疡复发：埃索美拉唑镁肠溶片20 mg + 阿莫西林1 g + 克拉霉素500 mg，每日二次，共7天。禁忌症 已知对埃索美拉唑、其它苯并咪唑类化合物或本品的任何其他成份过敏者。
注意事项
1.当出现任何报警症状(如显著的非有意的体重下降、反复的呕吐、吞咽困难、吐血或黑便)，怀疑有胃溃疡或已患有胃溃疡时，应排除恶性肿瘤，因为使用埃索美拉唑镁肠溶片治疗可减轻症状，延误诊断。
2.长期使用该药治疗的患者(特别是使用1年以上者)应定期进行监测。
3.肾功能损害的患者无需调整剂量。对于严重肾功能不全的患者，由于使用该药的经验有限，治疗时应慎重。
4.轻到中度肝功能损害的患者无需调整剂量。对于严重肝功能损害的患者，应采用的埃索美拉唑镁肠溶片剂量为20mg。
5.对驾驶和使用机器能力的影响尚未观察到。
6. 目前无妊娠期使用埃索美拉唑的临床资料可供参考。给妊娠期妇女使用埃索美拉唑时应慎重。
7.尚不清楚埃索美拉唑是否会经人乳排泄。也未在哺乳期妇女中进行过埃索美拉唑的研究，因此在哺乳期间不应使用埃索美拉唑镁肠溶片 。
8.尚无在儿童中使用埃索美拉唑的经验。
9.老年患者无需调整剂量。
10.埃索美拉唑治疗期间酮康唑和依曲康唑的吸收会降低。与地西泮、西酞普兰、丙米嗪、氯米帕明、苯妥英等)合用时，这些药物可能需要降低剂量。因此，苯妥英治疗期间，当合用或停用埃索美拉唑时，建议监测苯妥英的血药浓度。

Esomeprazole - Pharmacology: Suppresses gastric acid secretion by blocking proton pump within gastric parietal cells. Indication: Oral Short-term treatment of heartburn and other symptoms of gastroesophageal reflux disease (GERD); short-term treatment in healing and symptomatic resolution of erosive esophagitis; maintenance of symptom resolution and healing of erosive esophagitis; in combination with amoxicillin and clarithromycin for treatment of Helicobacter pylori infection and duodenal ulcer disease to eradicate H. pylori ; reduction in occurrence of gastric ulcers associated with continuous NSAID therapy in patients at risk of developing gastric ulcers; long-term treatment of pathological hypersecretory conditions, including Zollinger-Ellison syndrome.
Unlabeled Uses Non-GERD dyspepsia; Barrett esophagus; stress ulcer prophylaxis. Esomeprazole is in a group of drugs called proton pump inhibitors. Esomeprazole decreases the amount of acid produced in the stomach. Esomeprazole is used to treat symptoms of gastroesophageal reflux disease (GERD) and other conditions involving excessive stomach acid such as Zollinger-Ellison syndrome.
Esomeprazole is also used to promote healing of erosive esophagitis(damage to your esophagus caused by stomach acid). Esomeprazole may also be given to prevent gastric ulcer caused by infection with helicobacter pylori (H. pylori), or by the use of nonsteroidal anti-inflammatory drugs (NSAIDs). Esomeprazole is not for immediate relief of heartburn symptoms. Esomeprazole may also be used for purposes not listed in this medication guide. Esomeprazole is a compound that inhibits gastric acid secretion and is indicated in the treatment of gastroesophageal reflux disease (GERD), the healing
of erosive esophagitis, and H. pylori eradication to reduce the risk of duodenal ulcer recurrence.
Esomeprazole belongs to a new class of antisecretory compounds, the substituted benzimidazoles, that do not exhibit anticholinergic or H2 histamine antagonistic properties, but that suppress gastric acid secretion by specific inhibition of the H ATPase at the secretory surface of the gastric parietal cell. By doing so, it inhibits acid secretion into the gsatric lumen.
This effect is dose-related and leads to inhibition of both basal and stimulated acid secretion irrespective of the stimulus. Indications: Adult: PO Erosive oesophagitis 40 mg once daily for 4 wk, up to 8 wk if needed. Maintenance: 20 mg/day. Peptic ulcer 20 mg twice daily for 7 days. NSAID-associated ulceration 20 mg once daily for 4-8 wk.
Zollinger-Ellison syndrome Initial: 40 mg twice daily. Usual range: 80-160 mg/day. Gastro-oesophageal reflux disease 20 mg once daily for 4 wk. IV Gastro-oesophageal reflux disease 20 or 40 mg once daily for ≤10 days. Convert to oral therapy as soon as possible.
Esomeprazole is a gastric medicine. It is used to treat conditions caused by excessive acidity in your stomach, such as stomach ulcers and reflux disease (reflux is also known as "heartburn", in which food or acid from your stomach backs up into your mouth, leaving a sour or bitter taste). Esomeprazole treats these conditions by reducing the amount of acid your stomach makes. Esomeprazole may also be used together with other medicines to treat a specific type of gastric problem called Helicobacter pylori or H. pylori gastritis.
