适应症
主要用于治疗高血压与充血性心力衰竭。
副作用
临床副作用:
1 头痛、疲倦，眩晕，情绪或睡眠紊乱，痛性痉挛；
2 体位性或非体位性低血压(参阅：注意事项)；
3 少数病例皮疹；
4 胃痛，厌食，恶心，腹痛，味觉障碍；
5 已报道干咳与服用ACE抑制剂有关，其特点为持续性。但停药后干咳消失，如有上述情况，应考虑这种症状可能是由药物引起的；
6 极少见：血管神经性水肿(参阅：警告). 对实验室指标的影响:血尿素和血肌酐中度升高，停止治疗后可恢复。这种升高多见于合并肾动脉狭窄、利尿剂治疗的高血压和肾衰患者。
注意事项
由于该药含有乳糖，故禁用于先天性半乳糖血症。此病患者对葡萄糖和半乳糖吸收不良，或缺乏乳糖酶。在免疫抑制病人，有引起中性粒细胞减少／粒细胞缺乏的危险当以下述方式服用ＡＣＥ抑制剂时，罕见病例出现粒细胞缺乏和／或骨髓抑制：
1 大剂量给药；
2 多系统疾病（胶原性疾病，如：系统性红斑狼疮或硬皮病）引起的肾功能衰竭患者、合并免疫抑制治疗和／或可能引起臼细胞减少的治疗的患者。预防这类事件最好的方法是严格遵守推荐的服用剂量。但是，假如这些病人需要服用ＡＣＥ抑制剂，应慎重评估危险／利益比。
药理毒理
药学特性： 血管紧张素转换酶抑制剂 培哚普利星一种血管紧张素转换酶抑制剂（ＡＣＥＩ）。血管紧张素转换酶可将血管紧张素转化为血管紧张素‖。血管紧张素‖具有明显的缩血管作用，并可刺激肾上腺皮质分泌醛固酮。
培哚普利可导致：
1 醛固酮分泌减少；
2 由于缺少醛固酮的副反馈，肾素活性增高；
3 长期服用，总外围动脉阻力降低，且优先作用于肌肉和肾脏血流，不伴有钠和液体潴留或反射性心动过速。与所有的转化酶抑制剂相同，培哚普利抑制强烈肽类血管扩张物质?缓激肽降解为无活性的肽类。对于低肾素水平或正常肾素水平的患者，培哚普利均能降低血压。培哚普利以其活性成分培哚普利拉发生作用，其他代谢产物无活性。
抗高血压作用的特点： 培哚普利可用于治疗各种程度的高血压：轻度，中度，或重度。降低卧位和立位的收缩压和舒张压。
培哚普利降低心脏负荷：
1 可能通过改变前列腺素的代谢，扩张静脉，降低前负荷；
2 降低总外周血管阻力，降低后负荷。
对心衰病人的研究显示：
1 降低左室和右室的充盈压；
2 降降低总外围血管阻力；
3 增加心输出量和提高心脏指数；
4 增加局部肌肉血流；
5 提高运动耐力。
药代动力学
本品口服吸收良好，且不受摄入食物的影响，给药后6-12小时血药浓度达至高峰，绝对生物利用度约为64-80%，表观分布容积约为21L/kg，终末消除半衰期约为35-50小时，每日一次，连续给药7-8天后血药浓度达至稳态，本品通过肝脏广泛代谢为无活性的代谢物，以10%的原药和60%的代谢物由尿液排出，血浆蛋白结合率约为97.5%。
用法用量
1 高血压:4mg一日4次；如有必要，在治疗1月后，增至8mg一日1次。高危病人和/或老年病人开始剂量:2mg一日1次；
2 肾功能障碍病人，根据肌肝清除率调整剂量:每日2mg，30～60m1/分；隔日一次2mg，15～30ml分；当日透析后给2mg，<15ml/分；
3 心力衰竭:起始剂量为2mg每日1次，维持剂量为4mg每日4次。

禁忌
在下列情况下禁用培哚普利：对培哚普利过敏；与使用ＡＣＥ抑制剂有关的血管神经性水肿病史；妊娠的４至9个月；哺乳。
在下列情况下不推荐使用培哚普利：
1 与保钾利尿剂、钾盐、锂盐、雌二醇氮芥合用；
2 双侧肾动脉狭窄或单肾肾动脉狭窄；
3 高血钾；
4 在妊娠的最初三个月和哺乳期。
药物作用
禁忌配伍:
1 保钾利尿剂(安体舒通，氨苯蝶啶，单独或联合)，治疗心力衰竭时除外(小剂量ACE抑制剂＋保钾利尿剂)，钾盐、高钾血症(可以致命，尤其在肾衰的病例，药物对血钾的升高具有协同作用)。除低血钾的患者，不要将补钾制剂或保钾利尿剂与ACE抑制剂合用；
2 锂:ACE抑制剂升高血锂浓度甚至达到毒性水平(减少锂的肾排泄)。如果必须使用ACE抑制剂，必须严密监测血锂水平并调整剂量；
3 雌二醇氮芥:血管神经性水肿的危险性增加。
药物过量
过量服用本品，能导致外周血管过度扩张，继而出现显著而持久的全身性低血压，可采取洗胃。引起明显低血压时，要求积极的心血管支持治疗，包括心、肺功能监护、抬高肢体、注意循环量和尿量。无禁忌症时也可采用血管收缩剂。静脉注射葡萄糖酸钙对逆转钙拮抗剂的效应有益。透析对本品无效。

Indications
Aceon is used to treat hypertension and coronary artery disease.
How It Works
The mechanism of action is unknown.
Dosage
Tablets (2 mg, 4 mg, 8 mg)
The recommended initial dose of Aceon is 4 mg per day. Dosages will be dependent upon the condition being treated. Only a doctor should decide upon the dosages a patient should taking and anyone taking this medication should consult with a physician on a regular basis.
Precautions
Aceon is contraindicated in people who have hypersensitivity to ingredients in Aceon.
Speak to a doctor if you have the following conditions: Congestive heart failure, and acute renal failure.
In patients who have a history of ischemic heart disease, a rapid drop in blood pressure may cause myocardial infarction or a cerebrovascular incident.
Anyone taking this medication should be under the close supervisions of a physician.
This medicine will cause harm to an unborn baby. Before taking Aceon, tell your doctor if you are pregnant or plan to become pregnant during treatment. Aceon passes into breast milk and may cause harm to a nursing baby. Consult your doctor before using this medication if you are pregnant or are breast-feeding a baby.
You should consult a doctor before using this medication if you are allergic to perindopril erbumine or similar medicines, or any of the other ingredients of Aceon.

Drug Interactions
Consult a doctor before taking Aceon if you are taking diuretics, potassium supplements, lithium, gentamicin, and injectable gold products such as sodium arithriomalate.
Before using this Aceon, tell your doctor or pharmacist about all prescription and non-prescription medications, including herbal products, minerals and vitamins you may be taking. Other medications may affect how Aceon works or may cause other unforeseen complications.
Adverse/Side Effects
It is not uncommon for Aceon to cause the following minor side effects: dizziness, cough, back pain, sinusitis, fever, dyspepsia, fever, and ear infections. If these side effects persist or become overly bothersome, contact your doctor or health-care professional.
If any of these side effects become bothersome seek medical attention immediately.
A very serious allergic reaction to this drug is rare. Get emergency medical help if you have any of these signs of an allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, or trouble breathing. This may not be a complete list of side effects and may be subject to change, consult with your doctor or pharmacist if you are experiencing any worrisome side effects.
Overdose
If you suspect overdose, contact your local poison control center or emergency room immediately.
