药理毒理

1.药理研究:培高利特为一典型的中枢神经突触后多巴胺受体激动剂,能明显抑制垂体前叶释放催乳素,降低正常和利血平化大鼠血清催乳素含量;降低大鼠脑内多巴胺代谢转化,从而降低脑内3,4二羟基苯乙酸含量,使黑质纹状体损伤的大鼠产生对侧旋转反应;增加小鼠自主活动能力和攀爬行为,引起大鼠和豚鼠的刻板化运动,以及对抗猴中脑背盖腹侧损伤引起的自发性震颤。
2.毒理研究:急性毒性雌性小鼠口服LD50为81.8mg/kg,雄性小鼠口服LD50为61.7mg/kg。 大鼠口服给药6个月长期毒性试验显示,本品能引起剂量依赖性的自主活动能力和应激性增加,体重增长速度减慢,能明显增加雌鼠卵巢重量,停药后,体重增加速度及卵巢重量均恢复正常。本品对大鼠的血液学、血液生化学及其它脏器及组织病理学等指标均无明显影响。狗口服给药三个月,无明显毒性反应。微生物回复突变试验,小鼠微核试验,体外培养人淋巴细胞染色体畸变试验,结果显示本品无致突变效应。小鼠致畸敏感期本品未引起致畸效应。
药代动力学

Wistar 大鼠皮下注射1mg/kg 14C标记的甲磺酸培高利特血药浓度很快达到高峰,并维持12小时以上。口服灌胃后,很快从胃肠道吸收分布于所有组织中,维持24小时以上。72小时后放射活性53.3%由粪便中排出,16.6%由尿中排出,67%由尸体中检出。
适应症

本品为一多巴胺受体激动剂,为复方左旋多巴制剂的协同药物,适用于:
1.帕金森病及帕金森综合征患者复方左旋多巴制剂疗效减退或出现运动功能障碍,如同开关现象等。也可用于早期联合治疗。
2.高催乳素血症。
用法用量

口服。
1.帕金森病:起始剂量为0.05mg/日,维持2~3日;以后在医生指导下逐渐增加剂量,每次增加0.05mg,加至最佳有效量,最大可增至0.2mg/日,或遵医嘱。
2.高催乳素血症:起始剂量为0.025~0.05mg/日,每2周调整一次剂量,极量为0.1~0.15mg/日,或遵医嘱。
不良反应

可出现恶心、呕吐、头晕、乏力、鼻塞、皮肤瘙痒、便秘等不良反应,若不良反应严重时须停药。 据国外文献报道,个别患者口服本品后,曾发生精神症状,位置性低血压;个别患者出现窦性心动过速伴房性早博。
禁忌症

对本品或其它麦角类衍生物过敏者。
注意事项

服用本品必须在医生严格指导下,从小剂量开始,逐步增加至最佳剂量。 严重心脏病及即往有神经病史的患者,使用高剂量时宜慎重。

Permax is belongs to the family of drugs known as dopamine agonist, Antiparkinsonism drug and commonly used for the treatment and prevention of Parkinson's disease, a disorder of the nervous system that causes difficulties with movement, muscle control, and balance. Permax/Pergolide works by acting in place of dopamine, a natural substance in the brain that is needed to control movement. Permax is being manufactured by Eli Lilly and Company and is available in 0.25mg Tablets.

What is this medicine?

PERGOLIDE (PER go lide) is used to control the signs and symptoms of Parkinson's disease. It helps to improve muscle control and movement difficulties.
 
 
This medicine may be used for other purposes; ask your health care provider or pharmacist if you have questions.

What should I tell my health care provider before I take this medicine?

They need to know if you have any of these conditions:
•history of heart disease or heart valve disease
•low blood pressure
•an unusual or allergic reaction to pergolide, ergot alkaloids, or other medicines, foods, dyes, or preservatives
•pregnant or trying to get pregnant
•breast-feeding

How should I use this medicine?

Take this medicine by mouth with a glass of water. Follow the directions on the prescription label. Take your doses at regular intervals. Do not take your medicine more often than directed. Do not stop taking except on the advice of your doctor or health care professional.

Talk to your pediatrician regarding the use of this medicine in children. Special care may be needed.

Overdosage: If you think you have taken too much of this medicine contact a poison control center or emergency room at once.
NOTE: This medicine is only for you. Do not share this medicine with others.

What may interact with this medicine?

•medicines for mental problems and psychotic disturbances
•metoclopramide

What should I watch for while using this medicine?

Visit your doctor or health care professional for regular checks on your progress. It may be several weeks or months before you feel the full effect of this medicine.

You may get drowsy or dizzy. Do not drive, use machinery, or do anything that needs mental alertness until you know how this medicine affects you. You may experience flushing, nausea, vomiting, pale skin, or sweating before dizziness or fainting occurs. Do not get up too quickly from a lying or sitting position. Report any dizziness or related symptoms to your health care provider as soon as possible. Alcohol may increase dizziness and drowsiness. Avoid alcoholic drinks. Do not take any medications that cause drowsiness without first checking with your health care provider.

If you find that you have sudden feelings of wanting to sleep during normal activities, like cooking, watching television, or while driving or riding in a car, you should contact your health care professional.

Your mouth may get dry. Chewing sugarless gum or sucking hard candy, and drinking plenty of water may help. Contact your doctor if the problem does not go away or is severe.

What side effects may I notice from receiving this medicine?

Side effects that you should report to your doctor or health care professional as soon as possible:
•anxiety, restlessness
•chest pain
•confusion
•difficulty breathing or catching your breath
•double vision, or other vision problems
•fainting spells
•falling asleep during normal activities like driving
•fast, irregular heartbeat
•hallucination, loss of contact with reality
•lower back pain, or chest or neck pain
•mental changes
•palpitations
•uncontrollable movements of the arms, face, hands, head, mouth, shoulders, or upper body
•unusually weak or tired
 
Side effects that usually do not require medical attention (report to your doctor or health care professional if they continue or are bothersome):
•constipation or diarrhea
•difficulty sleeping
•headache
•loss of appetite
•nausea
•runny or stuffy nose
•stomach pain
•vomiting
•weight change

Where should I keep my medicine?

Keep out of the reach of children.

Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F). Throw away any unused medicine after the expiration date.
