达克普隆-Takepron（兰索拉唑）-抗酸及抗溃疡药

武田公司Takeda8月20日宣布日本厚生省批准兰索拉唑口服制剂lansoprazole，Takepron Capsules 15/Takepron OD Tablets 15用于预防非甾体抗炎药NSAIDs相关胃溃疡和十二指肠溃疡复发。
Takepron为武田公司开发的质子泵抑制剂，适应证为胃溃疡、十二指肠溃疡和反流性食管炎。NSAIDs主要处方用于疼痛和炎症的治疗，长期服用可能会引起胃或十二指肠溃疡，但停药可能使患者尤其是类风湿关节炎和骨关节炎患者的生活质量恶化。
对有溃疡史并需要长期服用低剂量阿司匹林或其他NSAIDs的患者而言，Takepron可以预防溃疡复发，是目前唯一一个获准此适应证的产品。
【药理作用】
为一新型质子泵抑制剂。本品由血液进入壁细胞内后，在酸性条件下被活化并与质子泵的氢钾ATP酶的巯基结合，抑制酶的活性从而抑制酸分泌。在体外，将组胺、氨甲酰胆碱、CAMP加入狗的离体壁细胞中时，酸生成显著亢进，给予本品后，可显著地抑制由这些因素导致的酸生成，抑制程度与本品浓度有明显的依赖关系；在同样的试验中，法莫替丁仅抑制由组胺刺激引起的酸生成。此外，兰索拉唑及其活性代谢产物具有与铋制剂相似的抗幽门螺杆菌的作用，对结扎大鼠幽门和前胃诱发的反流性食管炎，本品亦有明显的抑制作用。
【适应症】
用于胃溃疡、十二指肠溃疡、吻合部溃疡、反流性食管炎、Zollinger-Ellison综合症(胃泌素瘤)等。
【用量用法】
[口服]成人每次３０ｍｇ，１日１次。另外，在胃溃疡、吻合部溃疡、反流性食管炎的情况下，通常服药至８周，十二指肠溃疡则为６周。
【注意事项】
1.过敏症：有时出现皮疹、瘙痒等，在这种情况下要停止给药。
2.肝脏：有时可发生ＧＰＴ、ＧＯＴ、ＡＬ－Ｐ、ＬＤＨ、r-ＧＴＰ升高。如若发现异常时，要立即停药。
3.血液：有时出现贫血，白细胞总数减少，嗜酸粒细胞增多等。
4.消化系统：有时出现便秘、腹泻、口干、腹部胀满等。
5.神经系统：有头痛、困倦、失眠等。
6.老年人、孕妇慎用，哺乳期妇女及小儿禁用。
7.有药物过敏症病史及肝功能障碍的病人慎用。
【药物相互作用】
因同类药物奥美拉唑有延缓地西泮及苯妥英代谢和排泄的作用，故本品如和地西泮及苯妥英合用时应慎重。
【规格】
胶囊：每个胶囊中含有兰索拉唑１５ｍｇ，３０ｍｇ，为白色硬胶囊制剂。

Lansoprazole
Why is this medication prescribed? 
Lansoprazole is used to treat ulcers; gastroesophageal reflux disease (GERD), a condition in which backward flow of acid from the stomach causes heartburn and injury of the food pipe (esophagus); and conditions where the stomach produces too much acid, such as Zollinger-Ellison syndrome.
Lansoprazole is used in combination with other medications to eliminate H. pylori, a bacteria that causes ulcers. Lansoprazole is in a class of medications called proton-pump inhibitors.
It works by decreasing the amount of acid made in the stomach.
How should this medicine be used? 
Lansoprazole comes as a delayed-release (long-acting) capsule and granules to make a delayed-release solution (liquid) to take by mouth. Lansoprazole is usually taken once a day, before eating.
When taken in combination with other medications to eliminate H. pylori, lansoprazole is taken twice a day (every 12 hours) or three times a day (every 8 hours) for 10 to 14 days. To help you remember to take lansoprazole, take it around the same time every day. Follow
the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand.
Take lansoprazole exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.
The capsule should be swallowed whole.
If you have difficulty swallowing capsules, lansoprazole capsules can be opened, and the granules can be sprinkled on 1 tablespoon of applesauce, Ensure pudding, cottage cheese, yogurt, or strained pears and swallowed immediately. The granules should not be chewed or
crushed.
The capsules can also be emptied into 2 ounces (60milliliters) of orange juice or tomato juice, mixed briefly, and swallowed immediately. Rinse the glass with some additional juice and drink immediately.
For patients who have a nasogastric tube, lansoprazole capsules can be opened and the granules mixed in 40 mL of apple juice.
The mixture should be injected through the nasogastric tube into the stomach. Then the tube should be flushed with some more apple juice.
To use lansoprazole oral solution, empty the contents of a packet into a container containing 2 tablespoons of water.
Stir well and drink immediately. If any granules remain, add more water, stir, and drink immediately. Do not use liquids or foods other than water. Do not crush or chew the granules.
Continue to take lansoprazole even if you feel well. Do not stop taking lansoprazole without talking to your doctor.

What special precautions should I follow?
Before taking lansoprazole,tell your doctor and pharmacist if you are allergic to lansoprazole or any other medications.
tell your doctor and pharmacist what prescription and nonprescription medications, vitamins, nutritional supplements, and herbal products you are taking. Be sure to
mention any of the following: ampicillin (Omnipen, Polycillin, Totacillin), clopidogrel (Plavix), digoxin (Lanoxin), ketoconazole (Nizoral), theophylline (Theo-bid, TheoDur), and
vitamins containing iron. Your doctor may need to change the doses of your medications or monitor you carefully for side effects.
if you are taking sucralfate (Carafate), take it at least 30 minutes after lansoprazole.
tell your doctor if you have or have ever had liver or kidney disease.
tell your doctor if you are pregnant, plan to become pregnant, or are breast-feeding. If you become pregnant while taking lansoprazole, call your doctor.
What special dietary instructions should I follow?
Unless your doctor tells you otherwise, continue your normal diet.
What should I do if I forget a dose? 
Take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your dosing schedule. Do not take a double dose to make up for a missed one.
What side effects can this medication cause?
Lansoprazole may cause side effects. Tell your doctor if either of these symptoms is severe or does not go away: stomach pain diarrhea
Lansoprazole may cause other side effects. Call your doctor if you have any unusual problems while taking this medication.
What storage conditions are needed for this medicine?
Keep this medication in the container it came in, tightly closed, and out of reach of children.
Store it at room temperature and away from excess heat and moisture (not in the bathroom). Throw away any medication that is outdated or no longer needed. Talk to your pharmacist about the proper disposal of your medication.
In case of emergency/overdose
In case of overdose, call your local poison control center at 1-800-222-1222. If the victim has collapsed or is not breathing, call local emergency services at 911.
What other information should I know?
Keep all appointments with your doctor.
Do not let anyone else take your medicine. Ask your pharmacist any questions you have about refilling your prescription.
It is important for you to keep a written list of all of the prescription and nonprescription (over-the-counter) medicines you are taking, as well as any products such as vitamins, minerals, or other dietary supplements. You should bring this list with you each time you visit a doctor or if you are admitted to a hospital. It is also important information to carry with you in case of emergencies.
