ROFERON（INTERFERON ALFA-2A）
What is it used for?
Roferon is used in the treatment of chronic hepatitis C, low grade non-Hodgkin’s lymphoma, advanced renal cell carcinoma, chronic active hepatitis B, chronic myelogenous leukaemia (CML) (and other bone marrow conditions causing platelet disorders), cutaneous T-cell lymphoma, hairy cell leukaemia and AIDS related Kaposi’s sarcoma.
Roferon may be used in combination with ribavirin for the treatment of chronic hepatitis C in patients who have relapsed to previous interferon monotherapy treatment.
Roferon treatment will not prevent a hepatitis C infected person from giving another person hepatitis C.
Contraindications and cautions:
• This medicine can cause flu-like symptoms such as aches and pains, fever, chills, sweating, headache, fatigue and nausea. These tend to be worse at the start of treatment and improve with continued treatment.
• This medicine has been associated with psychiatric side effects, particularly depression, that can occur at any time during treatment and even after treatment has finished. This medicine has rarely been associated with side effects on the eyes. You should have an eye examination before starting treatment. Tell your doctor if you experience any disturbances in your vision during treatment.
• This medicine may rarely decrease the number of blood cells in your blood.
• Your ability to become pregnant or father a child may be affected by treatment with this medicine. It is important to discuss fertility with your doctor before starting treatment.
Use with caution in
• History of depression or other psychiatric disorders
• Diabetes
• High blood pressure (hypertension)
• Decreased kidney function
• Decreased liver function
• Heart disease
• Decreased blood cell production by the bone marrow, causing lower than normal levels of blood cells in the blood
• History of disease caused by the immune system attacking the body's own tissues (autoimmune disease, eg autoimmune hepatitis)
• Overactive or underactive thyroid gland
• Psoriasis
• Sarcoidosis
Not to be used in
• Premature babies, newborn babies, infants or young children. This medicine is also not recommended for older children.
• Severe heart disease
• Severely decreased kidney function
• Severely decreased liver function
• Severely decreased blood cell production by the bone marrow, causing lower than normal levels of blood cells in the blood
• Uncontrolled epilepsy
• People with chronic hepatitis who have liver cirrhosis, or who are being or have recently been treated with immunosuppressive medicines
• AIDS patients with CD4 counts less than 250/mm3, or those with a history of opportunistic infections
Certain medicines should not be used during pregnancy or breastfeeding.
Side effects: While you are taking Roferon treatment the most serious side effects include:
• Mental health problems – Roferon may cause some patients to develop mood or behavioural problems. Signs of these problems include irritability (getting easily upset), depression (feeling low, feeling bad about yourself or feeling hopeless), and anxiety. Some patients may have aggressive behaviour.
• Blood problems – Some patients taking Roferon have a drop in their white blood cell and platelet count.
• Body organ problems – Some patients may experience lung problems (such as difficulty breathing or pneumonia) and eye problems that can cause blurred vision or loss of your vision.
These are serious side effects. Serious side effects are rare.
Tell your doctor if you notice any of the following and they worry you:
• flu-like symptoms such as fever, tiredness, chills, muscle or joint pain and headache (these symptoms can usually be relieved by paracetamol)
• loss of appetite, taste change or dry mouth
• tiredness or sleepiness (may be less troublesome if you have your injection in the evening or before going to sleep)
• dizziness
• nausea (feeling as if you are about to vomit), vomiting or diarrhoea
• heart burn
• slight hair loss (usually temporary and reversible after finishing treatment)
• mild or moderate abdominal (gut) pain
• rash
• dry or itchy skin
• weight loss
• night sweats
• temporary impotence
• reappearance of cold sores if previously infected with herpes virus
This is not a complete list of all possible side effects. Others may occur in some people and there may be some side effects not yet known.
Interactions: It is important to tell your doctor or pharmacist what medicines you are already taking, including those bought without a prescription and herbal medicines, before you start treatment with this medicine.
This medicine should not be used in AIDS patients who are taking protease inhibitors or reverse transcriptase inhibitors (except zidovudine).
The may be an increased risk of side effects if this medicine is used in combination with cancer chemotherapy or other treatments that can have effects on the blood cells.
Interferon alfa may increase the blood levels of the following medicines:
• theophylline
• zidovudine.
Other medicines containing the same active ingredient.

【性状】
本药是一种含有165个氨基酸的高纯度无菌蛋白质。分子量约为19,600道尔顿。它是利用生物遗传工程处理过、含有该种人类蛋白质基因密码的大肠杆菌株，通过重组去氧核糖核酸技术而制成的高纯度α-2a干扰素。外观呈无色透明的液体。每瓶含300万或450万国际单位的α-2a干扰素。
【药理作用】
本药具有天然的人类α-2a干扰素的特性。其抗肿瘤作用的主要机制尚未明了，但本药已显示在体外试验中对不同人体肿瘤有抗增生作用，对裸鼠之人体肿瘤移植有抑制作用。本药的抗肿瘤作用已在毛状细胞白血病患者和卡波济氏肉瘤中获得证实。
本药与特殊膜受体结合后，刺激细胞产生
(1)2',5'-寡腺苷酸合成酶，
(2) ds RNA-依赖性蛋白激酶，
(3) Mx蛋白等，这些产物可抑制病毒蛋白质的合成，病毒核酸密码的转录，并分解病毒的RNA，另外本药可以调节免疫力，影响细胞膜表面抗原的表达，也可影响很多免疫中间物质的细胞膜受体表达，影响细胞毒性T细胞和自然杀伤细胞及B细胞作用。
【药代动力学】
健康人3600万iu肌肉注射，峰浓度为1500-2580 pg/mL(平均2020 pg/mL)，平均达峰时间为3.8小时。皮下注射后，峰浓度为1250-2320 pg/mL(平均1730pg/mL)，平均达峰时间为7.3小时。肌肉注射后，吸收80%。
【适应症】
慢性活动性乙型肝炎，丙型肝炎，尖锐湿疣，毛状细胞白血病，慢性粒细胞性白血病，多发性骨髓瘤，非何杰金氏淋巴瘤，与骨髓增生疾病相关的血小板增多，卡波济氏肉瘤，恶性黑色素瘤，肾细胞癌。
【用法用量】
慢性活动性肝炎450万iu，皮下注射，每周3次，持续4-6个月。一个月后病毒复制标志没有下降，可考虑增加剂量。3-4个月后未获改善，可考虑停药。丙型肝炎300-600万 iu，肌注或皮下注射，每周3次，持续12个月。
【不良反应】
流感样症状如头痛、发热、寒战、乏力、倦怠、肌痛、食欲减退、呕吐等。白细胞或血小板减少。血压低，心脏节律不整或心悸。少见嗜睡，体重减轻，脱发。
【禁忌症】
对本药或其它干扰素制剂有过敏史者，有严重的心脏疾病患者，有惊厥或中枢神经系统功能损伤的患者，严重肾、肝或骨髓功能不全者。
【孕妇及哺乳期妇女用药】
妊娠、哺乳妇女慎用。
【药物相互作用】
α-干扰素改变细胞代谢，故可能改变其它药物的活性，但其临床意义尚未明确。
