药物剂型
1.片剂：25mg，50mg，100mg；
2.颗粒剂：100mg(10%)，500mg(5%)。
药理作用
为非典型抗精神病药。本品对中枢多巴胺D1，D2具有亲和力，也可与5-HT2，α-受体和组胺H1-受体结合，并抑制去甲肾上腺素的再摄取。
药动学
本品可从胃肠道吸收，2～3h可达血药峰值。进行广泛的首过代谢，成为活性相等的去甲佐替平和失活代谢物。在代谢中涉及P450CYP1A2和3A4。佐替平和去甲佐替平的蛋白结合率为97%。本品主要以代谢物随尿、粪便排出。t1/2约为14h。
适应证
用于精神分裂症所致的兴奋、激动、躁狂、激惹性增高、冲动行为、焦虑、紧张、多动徘徊等。
禁忌证
1.个人有或家庭有癫痫史者、对本品过敏者、孕妇、哺乳者禁用。
2.本品有促尿酸尿特性，因此，有痛风史或肾结石病史者禁用。
3.由于本品可能延长Q-T间期，有Q-T间期延长史者禁用。
注意事项
1.接受过脑叶切除术或电击疗法患者，发热患者及高龄者慎用。2.服药期间不能驾驶车辆或操作机械。3.肝、肾功能不全者慎用。
不良反应
1.常见：困倦、失眠、乏力、便秘、头晕、指颤、视力模糊、血压下降、心律失常、心电图改变。偶见：ALT、AST升高。
2.本品有可能引起Q-T间期延长。
用法用量
1.开始口服25mg，每天3次，根据效应每4天增加一次剂量，最高可达100mg，每天3次。在每天总量超过300mg时较易导致癫痫发作。
2.老年人、肝肾功能不全者应减量，开始25mg，每天2次，加量最大不超过75mg，每天2次。
药物相应作用
1.参见氯丙嗪。
2.其他可能延长Q-T间期的药物可增加出现心律失常的危险性。
3.合用氟西汀或地西泮可能升高本品的血药浓度。
专家点评
本品其作用比氯丙嗪、氟哌啶醇强。本品可使痉挛阈值下降，从而加重戊四氮的作用。对兴奋吵闹、激动躁狂、焦虑紧张尤为有效。对多动徘徊、情绪不稳、自语、痴笑、幻觉妄想、呆板行为也有较好的效果。
An atypical antipsychotic drug used for the treatment of schizophrenia. It is available as tablets on prescription only.
Side effects:
include weight gain, dizziness, low blood pressure on standing (which can cause fainting in some people), constipation, indigestion, dry mouth, a fast heart rate, headache, insomnia, drowsiness, and fever. Zotepine may also prolong a certain phase of the heartbeat (the QT interval) and cause blood disorders.
Precautions:
zotepine should not be given to those suffering from acute gout, to people who have had kidney stones, or to women who are breastfeeding. It should be used with caution in people with cardiovascular disease, a history of epilepsy or Parkinson's disease, liver or kidney disease, an enlarged prostate gland, or acute glaucoma, and in pregnant women. It should also be used with care in people taking other drugs that prolong the QT interval (since this may give rise to arrhythmias; see below) or that cause loss of potassium (such as some diuretics).
Interactions with other drugs:
Anaesthetics: their effect in lowering blood pressure is increased.
Anti-arrhythmic drugs there is an increased risk of arrhythmias if these drugs are taken with zotepine.
Antidepressant drugs there is an increased risk of arrhythmias if zotepine is taken with tricyclic antidepressants; fluoxetine increases the plasma concentration of zotepine.
Antiepileptic drugs: zotepine antagonizes the effects of these drugs in controlling seizures.
Atomoxetine there is an increased risk of arrhythmias if zotepine is taken with this drug.
Methadone: there is an increased risk of arrhythmias if this drug is taken with zotepine.
Ritonavir may increase the effects of zotepine.
Sedatives: the sedative effects of zotepine are increased if it is taken with anxiolytic or hypnotic drugs; diazepam increases the plasma concentration of zotepine.
Sibutramine increases the risk of adverse effects on the central nervous system and should not be taken with zotepine.
